Good imho - a little no frill

I am satisfied. At a basic level everyone follows the same protocols. Either I am not a candidate for advanced treatments beyond Lupron and Xtandi or they just don’t “go there”. I’m a 5-year since diagnosis Gleason 9 with multiple lymph node involvement, one near the heart. I had chemo in a German clinic without involvement with the military. When I retuned to Austin I began with private care and concluded they were simply using me as a resource for maximizing income. I did not accept their Provenge recommendation. Went to VA at that point and found a refuge. Recently changed providers from Austin to Asheville NC, which is much better staffed than Central Texas. I have never felt tempted to return to community care.

Sounds like Lupron, chemo and Xtandi are working for now?Did you have chemo before Xtandi?

PSA is low?

Scans show stability?

That would be good. I hope that's the case.

Yes, chemo immediately to get me off catheter. PSA has never been very low, as in approaching .0 Mets doing okay, no growth. I have comorbidity with IPF, which has progressed to the point of “the race is on”

Best of luck. VA Dropped the ball on me, but that was AZ VA when they were at their worst 6 years ago. I will now follow you and see how VA works out. Life Is Good, especially with a good team on your side.

I’m not very knowledgeable about any of this but remember reading about senolytic therapy, and a study with good results, in IPF (assume this is Idiopathic Pulmonary Fibrosis). As senolytic therapy is now being studied in cancer/prostate cancer, perhaps something you can look into, if you haven’t already. Best of luck.

If the D+Q combo works it will hit a disease like IPF, as tentatively indicated in the trial, as well as PC senescent cells. Fisetin is a possible alternative that is easier to get hold of and with a more favorable side effect profile but the first clinical trial is only due in September.
