PSA slowly going down.

Uro just called with my PSA results, going down they have no explanation for this. My PSA has been going up slowly since 11/2018. PSA reached 0.23 in 7/2021, in 10/2021 PSA was 0.22. Today my PSA was 0.19. I have been on lupron since 3/2017, 3 month injections. They could not give my a reason why this is happened. Any thoughts.

No doc or expert here but those are all pretty low PSA numbers. The fluctuations could simply be the naturally occurring variation from test to test

Your PSA is stable at 0.2 if you round the numbers.

Those are not "going down." You are trying to believe the test is more sensitive than it is.

No worries as long as it's going down that's a good sign and if it does go up it can go up a couple points before you start really worrying about it so you're on the right track don't worry about it

You're doing well on Lupron. Keep up the good work. About a decade ago my PSA kept going down in small increments for 3 1/2 years. Ride Lupron for all it's worth.

IMO, if you are on Lupron alone and have no other symptoms, you are still on an absolutely great ride with this singular treatment and your cancer is castrate-sensitive. With respect to testing, there are too many small influences possible, especially at this scale. Based on my own hopes/fears on trending over the last 5 years, I suggest not getting too vested until a real trend can be identified.

I'll soapbox. We all need to remember that test values are not just numbers - they have units (PSA unit is nanograms per milliliter). Volumetric units are affected by both real changes in us and in the testing stream. Hydration is one of the most basic examples of how these volumetric units can change without the total amount of a specific substance in our bodies changing. In the testing stream, a change in technician, machine, or machine calibration can affect the outcome.

Honestly, I hope that some unknowable and idiosyncratic process is driving down your PSA and it gives us even more confidence that your cancer is castrate-sensitive and you have many, many years left! Good luck! - Joe M.
