During my consultation with Dr Aparicio, we talked about checkpoint inhibitors. She said the thinking about why prostate tumors are cold, is that Tumor Associated Macrophages loeer T cell infiltration. I decided to dig into this and came across a summary of the Tumor Immune Microenvironment. Here’s the article for anyone that’s interested in some light reading over the holidays.

Thanks you for sharing your journey with us. You seem to have learned a great deal in a short amount of time, and it's very enlightening to get the perspective of someone who is pushing the envelope.

You might be interested to know that there's a study at UCSF (clinicaltrials.gov/ct2/show... in which Dr Rahul Aggarwal 'primes' the t-cells to be sensitized to the cancer with a single dose of Lu-177 so that subsequent treatments with Keytruda can remove the checkpoint barrier. I'm in a support group with someone in the trial, and he says that they have some success (unfortunately, prior taxane treatment is a disqualification for the actual trial). Perhaps Dr Dorff might be willing to try it off-label . I've heard that Merck is fairly open minded about supplying Keytruda to clinical investigators, so it might be worth a try.

Dr A presented a poster at ACSO last summer, I can try to PM you a copy if you're interested.

Also, have you considered BAT therapy?
