Well more bad news!!! Docetaxel is not working. Going to ask for Jenteva not sure the spelling. That is fourth failed treatment. Zytiga, Lynparza, Keytruda now Docetaxel. So petrified this is the beginning of the end. Could really use some words of encouragement. Something has to work!!!!! 😭

Hang in there brother, there are so many treatments out there just finding the right one

I'm sorry about the progression. Jevtana is next for my husband too.

That raises the question...does he have neuroendocrine variant ...Has the doctor checked Neuro Endocrine biomarkers ...Chromogranin A, Neuron specific Enolase and Synaptophysin...and biopsy of met ..if needed. Look for other evidence of NE variant such as Osteolytic type bone lesion...high Lactate dehydrogenase (LDH)Once it is clear that he has NE variant..then go for platinum based chemotherapy. As per researchers, men with NE variant if treated correctly ..can live from 2 to 5 years...some even more.

Usually I heard the psa is very low with that! His is not.

That is true...But not always. Most NE variants harbor both Regular Androgen sensitive/resistant cancer cells and certain percentage of NeuroEndocrine type cells.My suggestion is to do work up for NE variant.. He may or may not have it but without work up, this cant be concluded.

Platinum based chemotherapies is a group of Chemotherapies which kills Neuro Endocrine Cancer cells...a prime example is Carboplatin.

Please don't be alarmed by self-styled "experts" on this forum who see neuroendocrine PCa everywhere and make up cancer biology. Very little of what they say is true. There are many aggressive types of PCa that are not neuroendocrine, and they may respond to different treatments.

He has not responded to anything thus far . Zytiga, Lynparza, Keytruda. With the Docetaxel his psa just keeps going up. He just had his fourth round. Spoke with his local oncologist yesterday. He said if someone doesn’t respond to Docetaxel it ususally is a poor prognosis. I have not yet heard from Dana Farber Dr Beltran. I want to see if she feels the same. I want to know where we go from here. I very concerned to say the least.

Cabazitaxel was specifically approved for patients in whom docetaxel failed.

Carboplatin and Jevtana helped me... It was a regiment of six treatments every three weeks. In my case it stopped the tumor growth. The only issue was the side effects - fatigue, loss of taste, and basically feeling crummy. That said, most guys my age feel crummy. ;-) Good luck!

You could consider treatment with Lu 177 PSMA, Ac 225 or Thorium. There are clinical trials in the USA:

Lu 177 PSMA treatment has shown to prolong life in heavy pretreated patients and it may be more effective than Cabazitaxel. Side effects are minor unless there is extensive diffuse infiltration of the bone marrow by the cancer.

These treatment can be also be obtained outside the USA mainly in Europe and India, if financially possible.

I've had good results from Jevtana (cabazitaxel), My PSA has stayed down much longer (nine months so far). Compared to docetaxel I had less hair loss but more nausea and greater changes in taste perception.

Docetaxel didn't work for me either, but so far jetvana and carboplatin are working well, my last PSA reading dropped 40%. There are many treatments available, the trick is finding the one that works for his type of cancer. Be persistent, be happy, think positively and enjoy every day you have together. Toss worry out the window.
