What is the chance of progression post castration surgery?

I posted a few months ago that I was considering the surgery. I did have the surgery in July. My MO supported my idea and stated that it would have the same effect as ADT. He also stated that it is not a cure. The reasoning for me to get the surgery was the fact that my MO said that I could not take my foot off of the brake when it came to ADT. I would be on these drugs the rest of my life. I have been in this fight almost seven years, as I approach 60 this year. My question is, what are the chances of progression with stage 4 HSMPC Gleason 9 ? Are there guys out there who progressed and are there those who did not?

Respectfully,

If you have metastatic prostate cancer (stage 4) there is 100% chance of progression if you live long enough. When the progression occurs is not as easy to predict, but there are general statistics available.

On the positive side, there are many treatments available to slow the progression down and give you as much life as possible. But as your doctor said, it's not curable.

Testosterone levels after surgical castration are usually 20 or lower and most probable lower than using Lupron or similar. I don't understand what is your concern about having an orchiectomy ( I assume that for surgical treatment you mean orchiectomy).

Mine stayed below 12 since the first and only lupron given 5 years ago. MOs call this situation "free lupron".

You will progress. debulking the prostate does not replace ADT.
