Update on our Cabazitaxel treatment, Any Experience/suggestions pls?

Hello Everyone!

We are currently on Cabazitaxel (post Abireterone->Docetaxel(10 cycle)). Already completed 5 cycles, 6th scheduled for 3rd June 21.

PSA (Before starting Cabazitaxel on 1st Feb 21) : 15

PSA (After 3 cycle on 5th April 21) : 13.64

My Father is tolerating it well without any issue but also he is in pain medicine since 6 months (and Quarterly Zoledronic acid Inj).

Germline BRCA : -ve, MSI -> Not high

Somatic BRCA with HRR test got failed due to poor DNA Quality (3.2 yr old Biopsy block)

Dr suggested for fresh biopsy in the future or on disease progression.

Any experiences/suggestions on Cabazitaxel, how long and well it works to control the disease?

You might want to discuss adding carboplatin, as in this trial:

He has bone and Lymph node Mets (marrow involved) but not any visceral Mets and neither any of BRCA/HRR genes came +ve (As somatic BRCA/HRR test failed on old biopsy blocks).

Do u still feel, Adding Carboplatin would be more effective in our case?

The trial did look at certain biomarkers. BRCA/HRR did not make a difference. But they defined a type that they called "aggressive variant PC" that made a big difference on whether the cocktail was effective. The aggressive variant was positive for mutation or loss of at least 2 of the 3 tumor suppressors: TP53, RB1, and PTEN
