Latest PSA and Testosterone

RP in Nov 2020, Gleason 8 with positive left lymph node and infiltration in the prostate bed after MRI with contrast. Bone scan in February was negative.

Jan 28th...PSA 8.4 and T 13.3

Mar 8th...PSA 3.7 and T 12.2

April 8th..PSA 3.2 and T 12.2

I have Colonoscopy on April12th, then PMSA PT/CT on April 15th with an appt with OR on the 22nd. I'm anticipating that radiation and ADT/Lupron will commence after the appointment and further investigations dependent upon the results of the scan.

Definitely whole pelvic radiation and 2-3 years of ADT, if no distant metastases are found on the PET scan.

Thank you for your advice and it coincides with what I have read and discussed with my GP. Your help and direction are invaluable.

Have their been definitive studies comparing the curative rate of 2-3 years of ADT vs. 6 months? Do you happen to know what that difference number is? 2-3 years is a long time. A friend of mine just finished 2 years and he sounded very discouraged.

I had a node positive Dx - in other words a spread to 2 'hot spots' in the lymphatic section - in the pelvic area - I was also G9.

I had ADT for about 18 months - then went on an ADT vacation - for over 2 years.

Then a BCR occurred and a PSMA/PET san revealed extensive spread in many lymph nodes - confined to the pelvic and abdomen areas.

Those 'other' nodes never showed up on earlier scans, but they were there - just 'too small' to be seen.

That points to 'regional pelvic radiation' treatment' + 2 years of ADT to TRY to get them all.

I don’t have my reference paper just now as I’m traveling. But I recall that only for those with very low pre-treatment PSA was 6 months ADT adequate with the pelvic RT (<0.4 I think). So that is not applicable to you and you should try to hold on for the two years. Try adding an estradiol patch to help with SEs.
