How effective has xtandi been for anyone after firmagon (or other adt drug) became less effective with psa rising? Also, is the tiredness symptom debilitating?

It is only given along with testosterone-stoppers like Firmagon or Lupron. The way to see if the fatigue is too much for you is to try it.

By the time I went on xtandi I had been on zytiga and casodex. I didn't have any problems with xtandi.

Hello, I have been on ADT drugs for 7 years and 8 months. Mostly Zoladex (Goserelin) but one year of Firmagon when PSA started to climb. I started Xtandi (Enzalutamide) in September 2017 three years and four months ago. Take the pills at night to combat fatigue. Some brain fog (could be old age. I am 75) but tiredness definitely not debilitating. Hope that helps.

If you PSA is going up during ADT , the cancer most probable is castration resistant. If there is not evidence of metastases, this link has the efficacy of enzalutamide in non metastatic castration resistant cancer:

Hi, I've been on Lupron since Dx in 1/2015 with GL 7, PSA at 840. 2. Did 15 Taxotere chemos that year from CHAARTED trial, hit a nadir of 0.1 the Summer of 2017.Had to do Abiraterone in Jan 2020. Over about 9 months the PSA never got below 4.1

Started Xtandi and PSA went from 9.2 to 8.9 and then 9.6

So, these new meds had almost no effect on PCa. Must be castrate resistant 😭. But, I made 6 years despite data saying only a 28% chance.

Axumin scan Nov 2020 showed 2 x 1cm high uptake spots in pelvic lymph nodes.

Gonna rechallenge with Taxotere and maybe if MO agrees, to do a BAT, if no significant drop .

No excessive fatigue with Xtandi, still able to work as a DDS.

I try to do daily PT(jumping jacks, crunches, mtn climbers, push ups, and weight lifting)

For ME Xtandi has worked since 5/2019. PSA at stage4 Dx (fully metastized), in Jun 2018 of 1303+. Leupron since 2018 dropped PSA to 1.2 in a few months till it started spiking back up to 9.1 when Xtandi added and last PSA .4 so currently working for ME.

Fatigue is worst side effect. Means I get more sleep and exercise.

Switched to Xtandi from zytiga. I take it in the evening after dinner. It seems to be much better than in the morning. PSA at .4 and fatigue not bad. I did have some brain fog initially. It seems to have gone away though or maybe I can’t remember. 😀 what I do know is the bone pain is gone and that’s amazing. Best of luck.
