Jevtana and rising PSA

My husband just finished jevtana 8 sessions and his psa is 91. Anyone else have this? The drde talked about a drug that what’s next?

He is going to do scans on the 20. The fr talked to him about a new drug that radiates on the inside. Do you know what he is talking about

Xofigo is the only approved radiopharmaceutical. It is for bone metastases only. There is another radiopharmaceutical, Lu-177-PSMA-617 that is in clinical trials now.

He did Xofigo and it failed.

Are you in Cupertino? UCSF has a few PSMA-targeted radiopharmaceutical trials. Stanford has a trial of a bombesin-targeted radiopharmaceutical.

Yes I am. Thank you. I don’t even know if my husband is interested in doing anything further. It’s upsetting to do 8 rounds of chemo and have your psa rise

Yes, it must be incredibly frustrating. The cancer does become taxane-resistant eventually. May I recommend talking to Rahul Aggarwal at UCSF? Not only is he lead investigator of the Lu-177-PSMA trial there, he is working of medicines for men that have been heavily pre-treated.

Just to tag on, would Lu177 treatment be helpful? My husband has been to India twice for Lu177 and is undetectable. He goes to India tomorrow for the last, third round.

Guides 12 & 13 of the NCCN Guidelines for Patients Advanced Prostate Cancer have a few drugs that you might still discuss with his doctor(s). Perhaps having some genetic testing of both inherited and tumor samples might also help in deciding "what's next" in rarer cases.

If you feel that he might not be interested in doing anything further then perhaps he might consider some complementary supplements that are relatively inexpensive and non toxic.

You might also be interested in this PUB MED case report. It's about a terminal pancreatic cancer patient who had to stop his chemo due to treatment intolerance and started taking two supplements instead.

I know that patient had pancreatic cancer and not prostate cancer, but if there's anything that "might" be able to help with that truly dreadful disease then it just might be worth trying for advanced prostate cancer too.

One of the supplements that patient took was Essiac which is an old herbal remedy that's been around for almost a hundred years.

I'm the first to admit that there's no peer reviewed scientific evidence to show that Essiac is effective, but I've been taking Essiac tincture together with CBD oil for almost 4 years and am very happy with my results so far.

When I stopped ADT in early 2017 due to severe side effects my oncologist assured me that my PSA would keep continually rising and my mets would spread further.

However, almost 4 years later my PSA is virtually the same as when I was first diagnosed December 2016 and my 2020 PSMA PET CT scan was better than my December 2016 PSMA PET CT scan.

I still see my oncologist every 3 months so he can keep a track of my progress.

He is amazed how well I'm doing as it's over 4 years since I was first diagnosed. He tells me to just keep doing what I've been doing because it seems to be working for me.
