Does castration effectively knock down the PSA?

Going to see doc in the morning. Had 2 lupron shots. PSA still at 156. Came down from 300 with first shot but not coming down with second shot. Wondering if there is a more potent shot. I want to get castration but having a problem getting DR to perform procedure without going through every alternative. I don't really want to take the shots. Anyone relate?

What's the rest of your info like Gleason score, etc. I started with a PSA of 156 and have been on Lupron and a month later started Zytiga and 2 months after that I started radiation. My PSA dropped to .5 in 3 months and <.1 in 6. Just a little anecdotal evidence of use of Zytiga but many smarter than me will chime in soon.

You could give the usual ADT drugs a try, i.e. casodex, zytiga, xtandi, etc.

If you are metastatic there should be no problem getting an orchiectomy. Also, if you are metastatic, you should also have docetaxel or Zytiga, Xtandi, or Erleada.

TA: Why does he have to be metastatic? His PSA is 156 more than 60 days after the commencement of ADT. Why does it matter where the PCa actually is?

Because if it is curable, only temporary ADT is needed.

March 2015, Gl 10, PSA 14+, age almost 65, extremely active, Urologist wanted to start ADT and I TOLD HIM ORCHIECTOMY and next day they were GONE. Took out the right half GL10 with Cryo and when I went back 7 months later for left half Focal Cryo plus immuno injection PSA < .1

IMO, Dr. Onik has been 30+ years ahead of the SOC mantra. He came in to see me ONLY MINUTES BEFORE my "standard cryo procedure on the Gleason 10 tumor" was to begin offering his EXPERIMENTAL injection as being the FIRST ONE, it took about 5 seconds to JUMP ON BOARD.

Only time will tell if I am a "lucky guy." Having started with castration, then treated, injected with IMMUNO and NOW biweekly Testosterone that is allowing me to have a fairly regular daily existence void of so many of the issues others have is humbling.

Two lupron shot into lupron for life club, I had orchiectomy instead of shot number three. Right choice for me. Now on 1/2 dose xtandi and no lupron to go with it.

Checking your total testosterone level should show you if the Lupron was adequate. Should be < 20 nG/dL. Equivalent to surgical castration. Some MOs favorDrugs over surgery because they don’t do them. Ask for a urology consult.
