PSA fluctuation can happen for many of reasons. The only way to find out if it's bad is after the scan. If the scan does not satisfy you, ask for a biopsy of the prostate. You don't want to sit on it and let it metastasize if you do have a high grade cancer. They did this with my father.

Did a biopsy of only 2 cores. Found that it was a gleason of 7. Told him with his PSA and gleason score to "not worry. It's low grade and won't spread quick". Let him go 8 months before surgery and then it metastasized.

I asked because this is a forum for men with advanced prostate cancer. Fluctuating PSAs before diagnosis usually indicate prostatitis.

Hi Deemond. Sort of similar frustrating experience here. 55yo.

PSA fluctuating over the past few years. Beginning in 2017-

2.1 2017

1.5 2018

4.6 2019 Feb. ( intermittent re-checking began due to big jump)

3.8 2019 Mar & 4Kscore 7%

2.2 2019 Apr

5.9 2019 June

4.2 2019 June again

All DREs normal

June had 3T mpMRI. Results showed nothing (except some disc degeneration) with a PiRads score of 1 (highly unlikely that there is the presence of high grade cancer) Apparently these MRIs have a negative predictive value of close to 90%, although the radiologist thinks even higher!

I understand your frustration...and your worry, if you have any. My urologist was quite encouraged, but I too never showed any signs of infections in my urines. He's thinking BPH (as was the doctor who interpreted my MRI - "uniform shape, large volume").

I am waiting a few months for a re-test PSA. Probably September or October.

Think about an MRI. My insurance paid for both - the 4K and the MRI. My urologist had to advocate big time for the MRI. They denied it the 1st time but he was able to argue/persuade at his second attempt. He is conservative inasmuch as NOT wanting to biopsy blindly and at this point he thinks it's counter-indicated to do so.

Good luck.
