My father (78) was recently diagnosed with prostate cancer by his urologist and we want to get a remote 2nd opinion from a research center like Mayo. We have the PSA score for this year (50+), PSA score for last year (~4), biopsy results with Gleason scores, CT soft-tissue scan (negative for lymph node spread), bone scan (likely spread), and bone CT scan.
What info should we gather to send to get the second opinion?

Ordinarily you foreword the complete medical record and make sure the things you mentioned are in there. Personally I‘d ask an oncologist to broaden the knowledge base . One from a renown cancer center with an acknowledged reputation. Guys here can probably give you specific questions and names to ask for if relevant.

I wouldn't suggest Mayo for a second opinion - they do not have a great urologic oncology team. My top list would include Eleni Efstathiou at MD Anderson, Emmanuel Antonarakis at Johns Hopkins, Evan Yu at SCCA, Matthew Smith at Dana Farber, Michael Morris at MSKCC, Maha Hussain at Northwestern, Oliver Sartor at Tulane and a few others. Get a second opinion from specific doctors, not an institution. These are oncologists who are working on the leading edge.
You would send your diagnostic and treatment records. If you can put together a condensed summary, that may give them a good snapshot of your situation.
They will require all the encounters with urologists and oncologists, plus the pathological reading of the biopsies (some places will require the actual slides), reports of all radiological and nuclear medicine tests (some places may require a copy of the CDs with the dicom images). If he had a radical prostatectomy they can require all the surgeon notes describing surgery and findings and the pathological report of the prostate histological studies (some places may require the slides).
