My Dad has stage 4 prostate cancer. He was diagnosed in late 2019. He is currently taking Lupron and Zytiga. His PSA has been steadily increasing each month. Has anyone else tried a different type of ADT therapy after there PSA increased? I’ve read that normally you would not try another ADT after one has stopped working.
I really appreciate any thoughts or comments!

Since there is generally cross-resistance bewteen second-line ADT drugs like Xtandi amd Zytiga, it usually makes more sense to go to a chemotherapy drug first, then go back and try another anti-androgen. This approach was recently proven in the CARD trial comparing second-line Cabazitaxel chemotherapy to the other of the two anti-androgens after the first became ineffective. The result was 3.7 months of radiographic progression free survival for the anti-androgen vs. 8 months for the chemotherapy.
I plan on doing chemotherapy when Zytiga becomes ineffective rather than trying Xtandi. The chemotherapies for PCa are quite tolerable for most. I've already done Docetaxel for 6 sycles so I know it's nothing to be afraid of.
Wow thank you so much for the fast response Gregg! I will read that article. The oncologist is currently recommending Provenge as well. Have you tried this treatment?
I haven't. I'm on the VA helathcare system and they don't offer it. Actually, it's only offered in the US. None of the countries that have nationalized healthcare currently offer it because the cost vs. benefit.
Hello Jason..... and bluline. Would you be kind enough to furnish us with bio info regarding your Father. i.e., Age? Location? Scores psa/gleason if available? Treatment center(s)? Doctor's name(s? All info is voluntary but it helps us help your Father and it helps us too.
If you do respond it may be a good idea to add that info to your Dad's/your homepage for future reference and for other members to view....
Thank you for the quick response. BTW It's a good thing you choose this site... many guys here with expertise and extremely helpful. Regards to you and your Dad.....
