Is this the end

So Mike has had this cancer for a little over 3 years. His last PSA had doubled from 12 to 24 in one month. He has small cell so this isn’t a great indicator. They did 1 cycle of yervoy/ nivo on March 9th. The last week he’s had a terrible cough, can’t clear his chest and has started to need a walker to walk. He also needs my assistance to get up from a sitting position. Latest chest X-ray showed the nodules in his lungs had grown from 16mm to 2cm and have increased in number. His doctor says the “plan” is to give another cycle of the yervoy/ nivo combo on 4/21. I’m not sure Mike will make it till then. The doctor gives me no ideas about if this is the end or not. I’m lost...

Since he has double negative prostate cancer, read this article and ask your MO about dovitinib. Mention “Right to Try” law and see if this could help along with Opdivo and Yervoy. Good luck!

Also I am sending my prayers and silent support, holding you both. You have been fighting with ferocity and devotion. Only you (together) can decide when it is time to stop, ok to let go of fighting so hard and focus on comfort. We honor and support you just the same.

You might want to review Section 5.6 "Immune-Mediated Pneumonitis" of the Prescribing Information for Yervoy at this link, and then discuss with his doctor(s) BEFORE getting the next infusion.

packageinserts.bms.com/pi/p...

(Maybe test to rule out a potential COVID-19 infection symptom, too?)

Please do not hesitate to "call in the cavalry" of others to support him and you in this difficult time if things continue to rapidly trend downward. Sometimes a good personal physician, a hospitalist, a hospital social worker, some palliative/hospice care specialists, etc. can do a lot to take much of the burden of suffering and pain from both a patient and a primary caregiver in such difficult times.

Hopefully, he may begin to recover soon, or during a break from treatment(s).

With the kids at home, there are always comparable dynamics happening, too. Getting some family support for you and them is/will be another area where "asking for help" sooner rather than later can have benefits.

Still, it is so hard. It is so painful. It is so sad. It is so tiring. It is so frightening sometimes.

One day, hour, 10 minutes at a time...

Big Hugs from all of us.

Hi Blair,

I just recently started reading your posts. I’m not on HU very often. I wanted you to know that you and your beautiful family are in my thoughts and prayers. Our paths are very similar. I am also married to a Mike with small cell pc. My Mike is now 47, dx at 44. We have two boys. It is a gut wrenching journey. Sending lots of love! 💙

Hi , my 53 year old husband is going through the same thing. My heart breaks for you. i just joined because we just found out from a liver biopsy that the cancer has new cells. Because of virus everything is over phone. He said it is neuroendocrine. Meeting with doc Friday to go over everything. Maybe you can help me with this. Stay strong my friend!!!

in reply tojmwife330

My husband was diagnosed with small cell(neuroendocrine) prostate cancer, found in a mass on his sacral nerve. He has had 6 radiation treatment to that lesion and to the right inguinal lymph nodes. He has had adenocarcinoma of prostate, had radical prostatectomy then radiation. His psa remained undetectable so we were thrilled. Then this. This mutated form of prostate cancer secretes no psa. There are very few men,a few wives, but few on these forums. Dr.Beltran at Dana Farber in Boston says it occurs in 20 to 30 percent of men who have been on chemical treatment of prostate cancer. Mind my French, but it is a bitch of a cancer, very ferocious and little effective treatments. I'm so sorry that your husband has this. Nothing good about it.

Hi, as wives we need to stick together . We started a three day treatment on Monday. Feel blessed that i do live in Boston. His cancer is in all the bones , liver and adrenal gland. I am thinking of your family ! It is such a hard road to go down. Stay strong and positive ! We need all the the positive energy around us that we can get right now!!!!!

Much love and prayers go out and upwards for you both.
