Docetaxel not working for me! PSA goes up after every dose. NOT sure it's worth the side effects. Any suggestions ?

Try cabazitaxel (Jevtana)

Add Xtandi, But watch for side effects and be ready to cut dose if they get too bad,

I went 9 cycles and ended up with permanent neuropathy to hands and feet.

I already have black and blue toenails and a big blister on my wrist.

Are you keeping your finger tips in an ice bath during the infusion? All you do is wear a pair of surgical gloves and hold your fingers in the ice. Take them out when it’s unbearable. And repeat.

For the toes and balls of the feet, try frozen gel pads.

Doing this kept me from having fingernail issues. I did not know about the gel pads, so I have neuropathy in the balls of my feet.

This really works!

Good luck on your journey! We are pulling for you!

Did the ice &gel pacs. Hands looked OK but still killed nerves. Feet lost 2 nails each and neuropathy in every thing below ankles,

I had same happen as you have had.

I was offered Cabazitaxel, as Tall Allen suggests, but I read a lot about Caba and it seemed like its not much better than Docet, and I was allowed to quit Docet after 4 shots and book in for Lu177 which worked much better with far lower side effects. I did have a 5th Docet shot while waiting for Lu177, but Psa went a little higher. My onco agreed with me that chemo probably would not work on bone mets and scans proved it.

But I had to wait a month before starting Lu177, and Psa went from 50 to 25, and docs didn't know why. But my chemo had net doubling of Psa from 12 before I began chemo.

Psa is now 1.5, rising but Pca is not a threat to bone strength. Chemo did not interfere with my PsMa expression, so the Lu177 could still work OK.

Yes I had short term positive response as well. They put me on Jevtana next. That was not successful either. Currently on Carboplatin and Etoposide, which has given a much better response. Will be moving to an immunotherapy clinical trial in March.

Sorry it's going badly. I did not know about icing. Lost 2 big toe nails and have foot neuropathy. Fingernails eventually went back to normal. Lost most hair everywhere. My psa did start to go down. (But I started lupron at same time.) Hope Jevtana or something works for you. Best of wishes.
