Update after Chemo

Started out with PSA of 2470 went down to 42 after Firmagon injection. Just completed 6 rounds of Chemo and 3 rounds of Zometa along with taking Bicalumide and Predisone. Current PSA is hovering around 100. MO was going to recommend Aberaterone w/ Presdisone but I saw a specialist at Georgetown Hospital who (based on a clinical trial 'Arches') is recommending Enzalutamide. Also eliminating Zometa, Bicalumide, Predisone. Anyone familiar with the Enzalutamide and the Arches trial? Appreciate any feedback.

Both Abiraterone and Enzalutamide are effective meds . PSA of 100 is not acceptable level and clearly indicates that still have enough cancer living in you.

The difference between these two drugs is that Abiraterone blocks all THREE sources (testicles, Adrenal glands and Cancer cells themselves) so I call it Broad Spectrum Anti Androgen

Enzalutamide blocks only one source of testosterone and that is testicles.

Side effects differ. Abiraterone can increase your blood pressure, lower your potassium level and increase your blood sugar. Enzalutamide can cause seizures and more tiredness.

If your doctor can manage your blood pressure and blood sugar well and if you eat lot of vegetables and fruits rich in potassium...you will do better with Abiraterone ..thats just my opinion ..ultimately you and your doctor have to make that decision. But you certainly need to knock down that 100 PSA.

14 months ago my psa was 1400 - 1600 and I started on Lupron with Xtandi ( enzalutamide ) and Zometa. My psa dropped 1350 points the very first month and has remained at <0.1 for the last 9 months. This regimen has caused me to experience heavy difficult side effects but my cancer seems to be responding well for now. My understanding is that Aberaterone has less SEs than Xtandi. Just my experience.

I had combined Docetaxel, Predisone and Prednisolone for ADT, my PSA went from 1386 to 0,028. I had subcapsular orchiectomy in Nov. The Chemo lasted 9 months and is on the rise again. Next treatment suggested is Raduim 223.

MO said to get PSA that low is way better than he expected, he said the Chemo would last from 9 to 15 months, so he was spot on with 9 months.

Hope you get the right drug combo to get you PSA down even more.

Not that many get diagnosed with high PSA (over 1000), we should start a group and call it "Sky High" slightly exclusive and probably a group most hope they don't need to join lol
