Maybe discuss reducing that to 5 mg once a day

You could discuss with your MO to reduce the dose to 5 mg of prednisone. One of the RCT of abiraterone (Zytiga) was done with this dose of 5 mg.

nejm.org/doi/full/10.1056/N...

Your doctors have to check for hypertension, edema and hypokalemia. If one of this complications appears when taking 5 mg of prednisone you could discuss adding Inspra (eplerenone). Inspra is an aldosterone receptor blocker. Patients treated with abiraterone (Zytiga) receive prednisone to control the increase in ACTH caused by low or absence of cortisol production by the adrenal glands. If ACTH is elevated it will cause an increase in aldosterone which could lead to sodium retention, edema, hypertension and hypokalemia. Blocking aldosterone receptors could help to control these potential complications.

Having slight swelling near ankles and high blood pressure 172/86.What tests should be done?

Of course glucocorticoids can cause edema, but so-called "pitting edema" in the ankles can be a sign of heart failure. Your blood pressure isn't well-controlled. Do you have a cardiologist?

Was he/she asked about the ankle swelling? Any changes in the BP meds?

5 mg daily...don't have swelling, but always hungry..so have to watch that or I will have weight issues.

Don’t reduce yourself, you have to step down. MO can help with schedule. Puffiness is a side effect..... I took 30 mg of Prednisone for six months during a clinical trial....... probably the biggest contributor to difficulties are any co-morbidity present. 5/10 mg daily is considered SOC as it is known to be within safe limits of the body and its reaction.

Similar to a “Z Pack” with its decreasing daily doses over ten days. Starting and stopping Prednisone should be under a Physician’s care.

Good luck.

I am on zytiga one 250mg tab per day. I was ordered prednisone 5mg but have not taken it because I have read that it may cause prostate cell mutations.

Please check and double check your source. Unless research has changed in the last 15 years, Prednisone, according to one of the eirld’s Leading researchers at the time, told me that Prednisone has cancer cell killing properties. I would think that the opposite would have had an adverse effect on me.

Good luck.
