Abiraterone and prednisone

I just started 1000 mg abiraterone and 5 mg prednisone ONCE daily. While reading online about this combo I found that 5 mg prednisone TWICE daily is the recommended dose. Do you think the pharmacy messed up the prescription? Should I call the MO about this?

5mg once a day is for hormone sensitive, 5 mg twice a day is for metastatic or castrate resistant. Sorry not sure which, maybe both, but I take 5mg once a day.

I wake up approx 5 am and take it then go back to sleep. I get up eat breakfast and take the Prednisone. I take 5mg so only once per day but it should be taken with food.

Yes, I think the pharmacist was wrong! In the instructions for the drug Zitiga prescribed 10 mg of Prednisone. Be sure to clarify this point and I wish you good luck in your treatment!

"Recommended dose: ZYTIGA 1,000 mg administered orally once daily in

combination with prednisone 5 mg administered orally twice daily. ZYTIGA

must be taken on an empty stomach. No food should be consumed for at least

two hours before the dose of ZYTIGA is taken and for at least one hour after

the dose of ZYTIGA is taken."

Why the variation between hormone sensitive and castration resistant? I thought the prednisone was only to counter the effect of Zytiga on cortisol. I am castration resistant and am on 5mg. My Onco started me on 10mg, then decided to trial 5 to see how I coped. I was fine so have stayed on that. Is this likely to cause a problem?

I am hormone-sensitive and have been on 5mg for years. Prednisone may have some side effects and PERHAPS that's the reason I am on 5mg and that your doc recommends the same.

Read the information provided when starting and a call from a clinical pharmacist made it clear that prednisone 10mg twice daily was necessary to counteract likely decreased cortisol production in the adrenal glands while Zytiga stopped production of androgens.

Agreed, but why is is different if you are hormone sensitive vs castration resistant??

I am guessing but being on long term steroids is not good for your bones and may effect other body functions or organs so they put you on the least amount that will do the job of replacing what Zytiga blocks and no more.

I believe it all depends on what your liver enzymes are doing. You should be getting monthly blood work. In my case 2.5 mg of prednisone once a day for 500-mg of abiraterone is sufficient thus far.

Thanks, I will be getting biweekly blood work for two months then monthly.
