It is not easy to see what Metformin does when given with Docetaxel. On the face of it, not much can be expected, as the Insulin systems are trashed by the Docetaxel (patients become diabetic for a while and Metformin cannot stop it) and the Metformin would just add to the poison load and gut biome wipe-out. The Metformin may help for some months after the chemo is stopped, by some limiting of sugar spikes (when the cancer cells are at maximum growth feeding on the sugar). This may be an explanation why Docetaxel fails for a lot of people - the weeks of uncontrollable high blood sugar levels are the best possible breading conditions for the cancer to grow very quickly.

There is some thought that Metformin has anti-cancer properties itself, but the effect can be explained by lower sugar spikes.

While most people tolerate Metformin, it is a viscous poison for many and can add greatly to a Pca victim's woes and mental state. There are alternatives like Jalra (Galvus).

The median survival time looks better to me (4.9 months better) especially when you consider that the median PSA of the Metformin group was a fair bit worse than the placebo group.
