Although my PSA levels have gone to 0.03 from 0.01 over 2 years, why do they fluctuate?

Its nature of PSA to fluctuate as cells are constantly releasing PSA and at the same time PSA is getting broken down and excreted. Fluctuation within a narrow range should not be cause of concern..it happens in everyone. Also,consider lab's margin of error.

I had a prostatectomy in 2017 and my PSA has gone.from 0.01 to 0.03. Should I be concerned?

Mark...I will repeat PSA every 14 days for two times...If it seems a upward trend..it should be some concern Just one reading does not make a trend...and its trend which matters in this disease.

This is a small fluctuation, I would not be concerned about it. What you need to watch is any steady progression over 5 or more tests.

If your pathology was good, only a distinct trend is important - at least 2 consecutive increases over 0.03 or an increase of over 0.05 in a year.

My gleason score was 7 (3+4) and no positive margins post prostatectomy. Also no nodal involvement.
