My husband has been on Zytiga Since Sept 1st . His PSA came down two points. Should we see a bigger decline or is this normal?

Good response. The chances are that the PSA will continue to go down. One needs to wait several months to see what the nadir could be. The time is variable from patient to patient.

He just came in and told me he’s been having back pain pretty much most of the day and I had no idea. Could your PSA still come down and cancer still spreading. I know it could be anything but you know how your mind wanders when something like this is going on

Can your PSA go down but your cancer still spread? I’m just asking because he just told me he’s been having back pain all day I didn’t know this.

Sometimes there is a symptom flare when starting a new therapy.

What causes the flare? Would chemotherapy cause a similar flare?

I think it's because dying cells release pro-inflammatory cytokines. See if an NSAID helps.

I've been on Zytiga for over 2 years and have pain on and off in various places. It's easy to automatically think that it's cancer progression, but there are many other possible reasons. My doctor told me that my skeleton has been damaged by the cancer so having some pain is not unusual. I also have some arthritis which complicates things. I think he also said that the cancer can make that worse.

I would suggest trying Ibuprofen or another NSAID, that generally works for me. If the pain persists, I would get imaging done to see what's going on.

Give zytiga a chance. Less than month is not a true test. His PSA is headed in the right direction.

Good luck

Hi Manohar11

I started Zytiga on 3 July 2020. My PSA bounced from 33, to 21 to 27.6 and last checked a week ago was 27.5. To complicate matters my liver numbers were in the high range. So my oncologist stopped me from taking any supplements other than my blood thinner, Vitamin D3 and Prednisone 10 mg a day since I am on Zytiga. He even stopped my cholesterol tablets.

He hit the nail on the head and my liver numbers became pristine when we repeated the PSA test after 2 weeks.

So I have figured out that since I am new to Zytiga, I expect my PSA to bounce around a bit and I pray it goes down as low as possible. I will be crossing my 3rd year since diagnosed on 9 October 2017 on 9 October 2020...phew that sounded better in my head than seeing it in print here...haha

I don’t have fatigue or any back aches...touch wood and keep my fingers crossed 🤞 but once in a while my mood swings gets the better of me. I understand the feeling when you said you are worried if any pain your hubby feels may be relational to his condition. Rest assured I feel the same. But my Lovely wife soothes my fragile nerves and fears by giving me the stare and glare, that somehow miraculously works and stops me from thinking every creak, pain or ache i feel or experience is related to my condition. She literally keeps me on my toes and good behavior. Great woman she is ❤️, without her support I would have disintegrated... Be that strong person for your hubby and remind him all’s well.

Take good care of yourself too and hubby and don’t let PSA numbers put fear in you. Trust the good doctors and the fantastic brothers on this forum who freely share their experiences and expertise and not forgetting the wonderful spouses and caregivers on this forum too 👏👏👏

Not to my knowledge and I’ve checked with my oncologist, but he said unless in the past there has been some pain with the core area of the back. Do get an X-ray at least. Do ask him to rest and stress less. 👍

My best to you and hubby ❤️

Hi,

It is too early to come to any conclusions. Please give ADT some time , at least 6 months.

The good thing is the downward trend.

Take care
