Weight Training and Advanced Prostate Cancer

I would appreciate some advice on beginning weight training. I'm seventy, was treated with robotic surgery followed by adjuvant radiation in 2013. PSA very slowly rising since 2017, not yet to 0.2. No additional treatment at this time but planning to develop the habit of weight training if ADT becomes necessary.

Personal trainers are skilled in their professions but I wonder if in this situation they are the ones to go to for a training protocol or another professional.

BTW when I used "training" in my search I learned a lot about potty training

Hi mikcon1. I know how hard it is to get started as Lupron made me bedridden for 2 years. The Livestrong program at many of the YMCA's have this 12 week, twice a week program. It really helped get me on track, and there is no cost to us. Not sure if it is the same everywhere because I believe they work from the generosity of donors but in my Y after you have completed the 12 week course they give you a 1 year free membershup to the Y for you and your family. I mean it really helped me.

I'd like to offer a suggestion along a different line.

Has anyone talked to you about salvage radiation? I don't know if it's appropriate for you. You may do fine without it. However if your PSA is still entirely confined to the prostate region, it may cure you.

Perhaps your doctors have already considered it and have a good reason for not doing it. However if it hasn't yet been considered I think it's worth considering. You'll want to talk to a very good radiation oncologist who specializes in prostate cancer in order to get the most informed opinion.

Thank you Alan I really appreciate your sincere and thoughtful concern and advice. I however had robotic surgery in 2013 and had undetectable PSA but chose also to have adjuvant radiation in 2013 after I had regained continence but prior to any PSA rise due to adverse conditions determined in the pathology. Some say no more radiation but others say yes but none will do anything without a scan that can determine if they can see a target. At my current PSA I've been told it's unlikely to "see" anything.

I would really love to find a way to delay ADT as long as possible but there is not a lot of info on recurrence following adjuvant radiation. I see a local MO this week for a consultation. I have a telephone consultation with Dr. Lam scheduled for next week also

My MO told me the best exercise to do while on ADT is weight training. “The guys that do it do great. The ones that don’t, drag in here. Hire a trainer and weight lift at least 3 times a week for an hour and a half. I work my butt off with a trainer and feel great. Actually lost weight, and gained muscle mass. It is possible!

I’ve found that using simple dumbbells and resistance bands 3 times a week have worked very well at maintaining muscle tone despite ADT for 5 years and counting. I have a workout room in the basement which makes it more likely for me to stick with the routine. The other days I go on brisk walks for cardio health. I also saw a physical therapist through a referral from my oncologist for some back pain, she taught me a lot of stretches and exercises to maintain a strong core to help prevent skeletal related events from long term ADT treatment and skeletal mets. I work these into my weight routine.

Keeping it simple and convenient is the key for me to sticking with the routine.

How well have you been able to maintain your muscle tone?? How about strength??...... I'm about to start ADT and really would like to lose as little of my current conditioning as possible..... I've worked a lifetime to maintain my weight and muscle tone and will not go gentle into losing it.....

I’m certainly not as strong as I used to be but muscle tone is probably better than it’s been in a long time, my wife even called me muscular the other day, I’m not sure about that but definitely have more definition in my abs and arms. I’ve got a little man boobage going for me but I’m 60 and when I’m at the beach I’m still probably better off than a lot of the guys I see my age in that department.

I watch the carbs I eat and have been taking metformin since dx and have lost about 20-25 lbs since dx so BMI is in normal range. Keeping the body healthy and fit is part of the battle and exercise almost always makes me feel better so I do it daily.

I've got the exercise for that boob problem -- called the 'fly'. Lie on your back on a bench. Hold dumbbells out to side and raise them up to meet in the middle. The 'bench press' doesn't do it -- gets shoulders and only the very top of chest.

Yep, been doing that one with both dumbbells and resistance bands, I honestly think I feel it more when using the bands, if your not using them give em a try. They’re great for traveling. I’ve been using GoFit Super Bands. The website is GoFit.net

What I do -- I hit LA Fitness gym 3 times a week. I jump on a stairclimber machine to start, for 10 minutes of intervals: 1 min challenging, 30 sec to a minute light, repeat. This is my least liked, huffing and puffing machine -- want to get it done and out of the way first.

Weight machines -- If you can do more than 10 or 12 repetitions maybe it's too light. Think opposites -- if one machine involves pushing upward, also use a different machine to pull downward. The fitness guy will explain it all. It's not onerous. You get out of the house and see different people. BTW, I am age 69. My fitness brag is that I can do five pull-ups.
