I have Gleason 9 Pca that spread to three pelvic lymph nodes determined at post surgical biopsy. I had RP in January, started Lupron shortly thereafter and start radiation next week.. My MO suggested that I have Foundation One genetic testing and I did. He indicated the results were that none of the mutations searched by the Foundation One test were identified. He characterized this as bad news and indicated that some of the major new drugs for Pca would not be useful to me because I did not have one of the key mutations. I am not sure what to make of this result. I would appreciate any insight anyone can shed on this.

I just had genetic testing. It was to be a indicator of which way my go for my next treatment, inhibitors or something else. The test was negative for BRAC mutations and in the meantime the other trial was stopped.

If you have Medicare, the drugs for these mutations are not really available. I got them only thru luck. My oncologist was no help.

If you have mutations this will indicate a poor prognosis if not treated with these new drugs. So without these mutations you just do not need these drugs.

Without mutations , there is also a poor prognosis. Just ask men who have burned thru standard care.

There are only three mutations (so far) that we have any drugs that provide enhanced response for prostate cancer. Carboplatin (and PARP inhibitors available in clinical trials only) enhances tumor response for those tumors that exhibit mutations in DNA repair genes. Keytruda is approved for those rare prostate cancer tumors that exhibit called MSI-Hi/dMMR. There are other drugs in clinical trials. Foundation One tests tumor tissue (called somatic mutations), but as new tumors progress, some may exhibit one or more of the treatable mutations. It also means that earlier tumors may not be susceptible.

So true as the first biopsy from my husbands lymph node was negative for mutations and second showed the rare msh2 which prompted him starting keytruda. He had one treatment, supposed to have second last fri but his liver counts went up so are delaying it to see if they go down told him NO alcohol,tylenol or statin for cholesterol to see if that affected count if not steroids will be given...
