I am just wondering if anyone knows whether zytiga or xtandi causes platelet production to drop.

Thank You,

Dan

I have never heard this - lack of testosterone can cause diminished production of red blood cells, of course. Chemo can cause thrombocytopenia.

As can cancer invading the bone marrow:(

Thank You Tall Allen for all the time you spend and expert advice you give!!

Most likely from the chemo and/or the radiation.

Did you get radiation to the pelvis?

I'm in Switzerland right now, fantastic! It's amazing here especially with my wife's friends and family, wish I could stay.

There are case reports about zytiga and thrombocytopenia. This is from Japan:

Thank You for that

According to the Zytiga presribing info, the COU-AA-301 trial of 791patients had no cases of thrombocytopenia indicated. Xtandi had a very small percentage, but almost the same as the placebo arm: In the randomized clinical trial, Grade 1-4 neutropenia occurred in 15% of patients on XTANDI (1% Grade 3-4) and in 6% of patients on placebo (no Grade 3-4). The incidence of Grade 1-4 thrombocytopenia was similar in both arms; 0.5% of patients on XTANDI and 1% on placebo experienced Grade 3-4 thrombocytopenia.

From the prescribing information:

Dan,

My platelets dropped as well post ADT/Zytiga from 257 to a low of 133. It's back up now to 183 after a few months. Not sure it's just a coincidence, or there's actually a correlation.

Dan,

I've been living with "low" platelet counts in the 110 to mid 130s for most of the last 5 years with no issues. My platelets are low due to my blood cancer (CLL) and not PC. I've never been treated for the CLL and have had only one 3 month ADT treatment.

As you probably know, many chemo treatments drive platelets down. I seem to remember that >90 to 100 is the range that will cause a delay in treatment regime. Corticosteroids are what were used in my sister's case (breast cancer) when her platelet count was low and she was on chemo.

I also take a lot of blood-thinning supplements, so about a year ago I started supplementing with viatmin K to boost my clotting factor. (Thank you Nalakrats) It also provides additional benefits for calcium uptake in bones. (and, therfore, not the arteries.) Not bad for a vitamin most people have never heard of. As always, check with your doctor(s) before starting any self-medication.

Thank You for this information, I may try some vitamin K. My platelets are in the 16 to 35 range currently. Presently I am in the iv chair waiting to get some specially matched platelets.

I finished my Xofigo infusions about 8 weeks ago and restarted Xtandi about 3 weeks ago. My Hemoglobin dropped to 7.2 and platelets to 46. I had 2 units of blood transfused last week so I could go on vacation (Bonaire) this week. The snorkeling has been great, although I am very limited to what shore dives I can accomplish - I walk with a cane and can't seem to manage even small waves and rocks/coral on shore. My fear is that the cancer may have entered the bone marrow...off to MD Anderson at the end of the month to see what's going on. Are we having fun yet??

My dad’s platelets go down a lot so they give home iron 2 x’s a day. Never needed a transfusion but it can be nerve racking. He’s on zytiga Firmagon and prednisone.
