Docetaxel vs Abiraterone?

I am just beginning treatment. On Lupron so far and soon to begin the next level. Docetaxel or Abirateroneb - Any thoughts about which one to do? Study results are similar but treatment duration and side effects are very different.

If there are multiple mets, you should consider docetaxel next.

That's where my team is leaning. Why cytotoxic chemo instead of hormonal therapy? I have stage IV with nodes and multiple bone mets, Gleason 8, PSA 145 before Lupron.

Because of the 17 month survival increase seen in men with multiple mets in CHAARTED. Also, it's done in 18 weeks at which time you can have Zytiga too.

Does Zytiga after docetaxel improve results? Intuitively it makes sense but are there studies?

There have been cases where docetaxel reverses resistance to Zytiga.

LATTITUDE study shows increased median life if one goes on abiraterone immediately after decetaxel. That's my plan this fall. The question is, what's next?

I was in your boat in Jan 18. I have been on Zytiga and Predisone since Jan; Lupron since April 18 and Xgeva (spread to bone t3 t8). Psa was 11,000!! I was dying, ate 1/2 cracker a day, weak could not move without severe pain as an idiot pulmonary doc kept me taking $21,000 in breathing tests from Nov 23-Jan2. No meds no cancer doc referral. Even with clear airways on mri and scoring 98% out of 100% on breathing tests, when I informed her I saw 2 cancer docs on my own she said falsely to cover her butt, I have to discharge you and you have COPD. I am 66, never smoked or drank, no sicknesses or surgeries. The woman was a Quack!

Anyway the good news is find a doc you trust, pray to your God, do your research, eat well, take vitamins ( I take Calcium D3 and K2). My PSA has been in steady decline and last week was at .3 ( less than 1 more than 0). Full body scan showed rib bones repairing itself, no cancer spread and the tumors at t3 and t8 are shrinking.

There is hope out there. Every case is different. Stay focused. Your life will change a bit but to see the smile on my wifes face and to kiss her lips; its all worth it. Have faith. Prayer, Medicine and Real Friends.

Do the chemo, it took my psa from 850 to 0.07, 3.5 years ago. I have yet to start Zytiga. Remember one thing, chemo kills without remorse. It's a breeze.
