2008 - proton beam, Gleason 6. PSA remained .001 until 11/15/15 -.017. Rose to 2.7 -12/16. Bone scan and MRI of prostate negative 1/17. PSA 9.1- 9/17 Ga-68 PET/CT showed hot spot in L seminal vesicle. Transperineal mapping biopsy negative for prostate and seminal vesicles- 11/17. Feraheme MRI - 3/18 negative for lymph nodes. PSA 19.2 on 6/18 - Axumin PET/CT 7/18 negative. Still watchful waiting.

Strange that PSMA Pet scan saw a hot spot in L seminal vesicle that biopsy could not confirm. Have you considered hormone therapy?

Recent Axumin Scan showed nothing. My overall health is excellent and am able to walk an average of 7.5 miles a day between walking on beach in morning and Golf in afternoon. Have played 83 rounds since coming back to The Oregon coast in mid April. Urologist wants another PSA in 3 months. Do not want the side effects of hormones. Have been taking 1.25 mg of finasteride for 15 years. Started with Propecia to stop hair loss.

Maybe the finasteride is shrinking it so that the biopsy can't detect it.

Interesting that they haven't found the spot. I did Proton initially in 2011, reoccurrence in 2016, HIFU in 2018. I did a 3T MRI that found a spot lower right hand corner of prostate. My psa held steady at 4.2 for a year, then 5.2, then 9,2, then 12.7 right before HIFU. It's one of the few options for treatment available post Proton as most surgeons aren't interested. Cryotherapy is another option. I was playing golf a week after the catheter removal from HIFU.

I did mine with Dr. Robert Pugach of Pacific Coast Urology in Los Alamitos, CA. There's another top guy near SF....can't think of his name of the top of my head. Closer to you.

I am a failed Proton patient and yesterday had my first hormone shot. I had tried a salvage treatment of Cryo and IRE but it did not work, also surgery was rules out due to the effects of my prior treatments, so with lymph nodes increasing in size and a suspect spot on my spine and confirmed cancer still left in what is left of my prostate I am out of options. The doc that did the Cryo put me on Avodart, that didn't seem to help with me.

Reasearch, then ask your Medical Oncologist to explain the term micro-metastasis.
