Anyone on Apalutamide (Erleata)

Anyone able to secure Apalutamide as a MCRPC patient? Understand from my On that it is a significant advancement. Am currently on Xtandi but PSA keeps going up (33) but all chemistries are normal? He suggests waiting until we can move to Apa.

Anyone have any backpain or leg weakness from Xtandi. I did very well on Zytiga for 5 years.

Welcome to The Twilight Zone...2 drugs one approved for nmCRPCA and the other for mCRPCA...You could have your Doc write a prescription for Apalutamide and if the insurance company turns you down, use the rejection letter for financial assistance. There is probably no rush to obtain Apalutamide approval for mCRPCA since there is not an unmet need. Last I read Apalutamide did not show a significant OS advantage over Xtandi so the mfg went for nmCRPCA. If you want to jump through some hoops try obtaining Darolutamide which is supposed to be superior to either one.

I am on Eligard for about one year and PSA going up. Now 4.0. Started Erleada a month ago and will get next PSA test in 3 weeks. Only difference in side effects were slightly more hot flashes of longer duration and more intense. All other side effects remained the same. I am in my 21st year battling PC.

Have taken Xtandi for 4 months but it ceased to respond. PSA shot up. Yes, had lower back pain, could hardly stand up. and fatigue. Still do after being off Xtandi since April. Started chemo with docetaxel in April. Not much effect on PSA but did reduce alcaline phosphatase (evidently relevant in advanced metastatic prostate bone cancer).

My husband is just 2 months behind you. Xtandi stopped working. We didn’t know because PSA was stable. He will start back on doctaxel on Friday.

Hi Lumbo, Similar to you, my husband is going on 18 years since diagnosed and is 75 today.

He had some leg muscle issues on Xtandi but walked them off. Lately cognitive issues had his MO Suggest hold off on Xtandi for awhile. His MO prefers Zytiga anyway so not sure what he will suggest on next visit.

My husband is asymptotic except for treatment side effects.
