I'd like to help others by posting about what I feel is a top provider of PCA/BPH diagnostics and treatment options.
My PSA was under 1 for many years and and a surprise jump to 3.5 then 6.5 but after a few months dropped to 3.5, and given Nalacrats (??) recent post re anti-inflamatories... my daily suppliments include all of his list at the end of :

Plus I take iodine (per Dr Brownsteins books for 10 yrs, 12mg --> 50mg / daily) which is strongly apoptosis'ic.

So I shouldn't have been too worried but I was... We are fortunate my wife and I do deep (very deep) study into best practices, tests, and Drs then go full bore and no $$$ limit re best path. We chose to skip the local Drs / clinics in Atlanta and go to what we felt was the best PCA / Diag clinic in the country. It happened to be in mid-Florida:
sperlingprostatecenter.com/...
They may have MRI centers in NY and CA as well. I traveled to FL.
MRI gadolinium contrast agent of choice: Dotarem
sperlingprostatecenter.com/...
and EU: ema.europa.eu/ema/index.jsp...
Sperlings diagnostic, biopsy, treatment process:
1. 3Tmp MRI (with or without contrast). I did without. They said if image had tumor suspects, they'd give me Dotarem contrast and put me back into the MRI. Thus you can go with zero contrast agent risk, and only take the risk if you have suspects in your 1st image...
$2500
Stop here if no suspected issues on the image.
2. If suspected tumor, then real time in MRI biopsy, in the MRI bore
Another $2500
At this point You have a high reliability diagnosis, with what I believe to be the best technology, lowest risk for missing tumor sites and fewest needles used (side effects).
The above can be done in one day if you start in the AM.
3. Treatment: back into the MRI for blue laser ablation or he mentioned ultra-sound ablation... I didn't study to this point...
========= Summary
Just my laymen view; real time MRI for diag, biopsy then treatment seems the most accurate, least invasive, least side effect path I've found, or read here. You be your own judge...
========= Our views re medical treatments, a general comment
I believe that even if we had few financial resources we'd not ask "how much" or even "who will pay" when we choose a health path. We choose to go see the best of the best Drs and get on airplanes for even dental procedures. My experience at Sperling and their tedious MRI believes that avoiding In Network fishing for servicers was a wise decision. My viewing my own 3T mp MRI being walked through the layers and structures by Dr Sperling I can't imagine getting my prostrate diagnosed via any other means. Hopefully there's other 3Tmp MRI PCA clinics??!!
This site (pay for) seems to mention Sperling vs other clinics:
Good luck!
