PC Invasion of My Liver

Just learned this week that my PC has invaded my liver. I was just about to have my 4th infusion for Xofigo (Radium 223). But my scan from last week showed this movement which made my continuing Xofigo not viable since it works on bone only. I'm standing here now waiting for the Oncologists, that are working my case, to decide what to do next. It doesn't look good. Anyone know what's next?

Have you had chemo yet? PSMA scan in Dec 2017 showed 10 lesions in my liver but my bones are clear. MO started docetaxal immediately and I have completed 5 rounds. 5 more to go, and MO said we will can do more if my tolerance remains ok. PSA has declined from 47 to 2.8 after 4 rounds, and I am anxiously hoping this continues.

But my case is different to yours (bone mets) we are all different and may require different treatment plans. Whatever happens next, good luck.

I did docetaxal with prednisone and completed 7 rounds before sideffects got to me. Good luck.

Sorry to hear about your liver mets.

Carboplatin chemotherapy has been effective for many people with liver mets, but it's a bit more difficult than Docetaxel from what I understand. There is also LU-177. There may be clinical trials you could get on. Where are you located?

I had cabazitaxel with carboplatin which worked on my liver mets for a few cycles.

I have been on two clinical trial since my last chemo treatment. I had exhausted all the standard FDA approved treatments.

Radioembolization for liver mets is a relatively new therapy that seems to work extraordinarily well. Then you can get back to Xofigo for your bone mets.

Allen can this radioembolization of liver mets be done by any RO, or is there a center of excellence that specializes in this treatment for liver mets?

They have to inject the radioactive beads in a specific place in the hepatic artery so that they will flow down to the place where the mets are. My understanding is that it is done as an outpatient procedure by a hepatologist working with a radiologist (who identifies the specific place in the artery). I think most major tertiary care cancer centers do it.

Here's a case report of a successful treatment of a patient with PCa and liver mets using radioembolization. It also contains a lot of general useful information.

Good Friday Morning Viperderf,

I have been in this battle for almost six years (please see bio for complete treatment history).

In 9/17, like you, my PCa had invaded liver and liver MRI indicated 20 lesions. Next was a CT guided biopsy of liver which confirmed PCa.

I began Docetaxel/Carboplatin chemo and completed 6 cycles, 3 weeks apart, on 1/9/18. PSA dropped from 10.8 to .4 after chemo. Subsequent liver MRIs indicated lesions either gone or smaller.

Currently on a chemo break with monthly blood tests and every 2 months liver MRI.

PSA rose again to 1.3 but rechallenge with Xtandi brought in down to .7 as measured this week.

Oh thank God ... a survivor! I was afraid they had just slammed the door in my face. Congrats on doing so well.

I have already done 7 rnds of Docetaxel w/prednisone so I'm not sure if I can go back there or not. But your story gives me hope. Thanks for that.
