Xtandi dosage

I am confused about the dosage of Xtandi, I'm sure I read some patients are taking 1 pill 4 times a day while others take 4 pills once a day. Did I read about these dosages on this site? I know what my oncologist said but I want it verified. Thank you.

I always took 4 pills once a day in the evening to fall asleep when it makes you tired. I believe 4 pills all at once is the recommended.

Thanks Dan, I was just hoping to have less side effects, I guess, by convincing myself 1 four times a day would do the trick...yes the recommended is all 4 at once. I was thinking the middle of the day for the dosage but side effects would probably be more noticeable. You take care.

The Full Prescribing Information for Xtandi has it taken as 4 capsules all at once, the same time each day.

I've been on Xtandi for about 8 1/2 months now. I happened to get into the habit of taking my 4 big white capsules at night, 2-3 hours before I go to sleep. It takes a good sized whole glass of water to get them down. I've found that if my chin is dropped, or if I push up with my tongue to allow them to touch the roof of my mouth, they can have a tendency to stick in my mouth, and be harder to dislodge and to swallow. What works best for me is to keep my chin up and to perch a capsule on the center of my tongue. Then "wash" it off and down with three swallows of water in rapid succession. Then another three quick swallows for each of the other capsules. And, finally, three good swallows at the end as a "chaser" for all four of them to wash all the way down.

As for side effects, I have not particularly noticed diurnal variations in side effects while on Xtandi. My own general feeling about this is that taking the Xtandi on a regular basis, as prescribed, leads to a relatively "steady state" of Xtandi circulating in the body, regardless what time of day the 4 capsules are taken. I most often feel some tiredness and light fatigue in the early afternoon, after having done some physical activities in the morning, despite the fact that I take my Xtandi later in the evening. My being on Lupron for 3 years and 9 months might have about 75% to do with that, too.

Without food. So I take it in the middle of the night when I get up to pee at about 2 or 3AM roughly.

Either is OK. I take it at bedtime. Unlike with Zytiga, I can have a midnight snack before I take Xtandi.

I initially took 4 pills once a day AM for about 9 months. Fatigue was bad and my onc changed it to 3 once a day PM - before bed. Reducing 1, and taking PM helped a little with fatigue. The 1 pill reduction did not affect the efficacy. I'm 3 1/2 years in and holding at 0.04. I don't take it with food. But dinner was usually within 2 hours of my dose. I hope this helps.

I take 4 pills at one time mid afternoon, no food. Very little side effects.

I took mine all 4 at once just as I was going to bed. I experienced no side effects. I attributed my slight fatigue to Lupron. Its more like I just don't have the drive to do as much as I used to.

I take 4 pills in the evening around bed time. Taking it early in the day affects my energy level.

The FDA approved dosage is 4 tabs a day. Sometimes if the side effects are too bad a doctor will tell you to cut back the number of pills. Don't move off the 4 pills per day without discussing it with your doc.

I am taking 4 caps at 10pm with 4oz of water. I moved from daytime to this time to decrease nausea. I have completed 31 days on Xtandi. Beginning seven days ago I have had regular swelling in my feet (more so on the left.) Not more than 1/16 of an inch, but still bothersome. My Onc people say this is not typical of their patients even though extremity swelling is noted as a side effect in the handout. I try to stay well hydrated during the day as I am working on my ranch. I drink less in the evening before bedtime due to nocturnal incontinence. (Some times I do not wake in time.) Swelling varies during the day from minimal to 1/16. Exercising the feet seem to reduce swelling - no swelling at night. Is Xtandi the cause or my hydration cycle or something else?

Thanks for the replies, I am back on track taking them mid-day, hoping I can still do my part time courier job with little side effects from the pills. Keep up the fight!

I was on Xrandi for 3 1/2 years 4 caps each night about 9pm. I always had 2 glasses of water.

Interesting, I am on a trial with xtandi + a mystery drug from Eli Lilly. For the first year I was to take xtandi, 4 pills once per day with one of the mystery drug. I had to take the xtandi without food. Nothing to eat 2 hours before or after. Now in the second year I am to take the same meds with or without food. They are testing the absorption.

Good luck on your trial, sounds interesting. I was confused on my taking, but have now righted the boat and am now smooth sailing with 4 Xtandi taken one time daily. I took them during the day and will continue at that time, I guess just to be able to track any side effects. The best to you on your trial.
