Hello! I am the coordinator for a Section 504 research project at Vanderbilt University, Special Education Department.

We are conducting a national survey with family members of children with disabilities and adults with disabilities who have had Section 504 plans at school.

You must be one of the following:

- A student (aged 12 or older) who has a Section 504 plan from school. The student's parent must consent for the student to complete the survey.

- A family caregiver of a child who had a Section 504 plan from school.

- An adult with a disability who had a Section 504 plan from school.

You will complete a survey. The survey will take 20-30 minutes to complete.

Complete the consent form here:

redcap.vumc.org/surveys/?s=...

Please reach out to me on here if you have any questions!

We are still looking for youth to be interviewed about their Section 504. They must be between the ages of 12-22 years old. If interested, please let me know and I will send more information.

Thank you!
