I'm new in this group and so happy to have people to share my questions My 7 years old daughter has ADHD she is on concerta 27 mg . she is more focus in class but at home she is agressive and angry and wat to mess with everyone. She has bad anger and sometimes scratchs us we have to isolate her to calm her down . We try to be calm to no interaat with her during the meltdow.
We have started intuniv 1 mg since 3 days
how long should it take to see an effect?
i feel so powerless i would like to help her and make also the life in the house more peaceful Her oldest sister is little traumatized and angry at her .. We are havins lots of discussion to help her to understant what is happening to her liltte sister

With Intuniv, you should start to see a difference right away, given it impacts blood pressure. If it's not working, you may need to increase the dose (from memory, 1mg is a very small amount).
We also use Concerta 27mg, and tried Intuniv, but found it made our son "wired" so he could not sleep. Adding a SSRI (for anxiety) really helped with emotional regulation and irritability. It might be something to consider if you find Intuniv does not help. We've also had success with Clonidine, a sister med to Intuniv, for improving sleep, which has improved mood.
We use 1 mg intunive ER for the same reasons with our 6 yr old. Everything I've been told and read says it takes a while to see full effectiveness of intunive. And I believe that. We tried 1mg AM and 1 mg PM to up the dose but I thought it made my 6 yr old tired and she complained about tingling hands and feet alot so we backed off to 1 mg again. I think it takes the edge off but wish it helped more.
Our doctor told us it would be about 2 weeks before we see a major impact with Intuniv. It may be she needs a different stimulant other than concerta. I have a 14 year old that has been on different dosages of concerta since he was 6. We just went to a psychiatrist, and he is trying Adderall due to the afternoon irritability. It is so hard finding the right combination.
My daughter (14 yo) is currently taking 2mg of Guanfacine extended release in the morning and 1 mg extended release at 4:30 pm and that seems to be working very well for her. Took a while to get the dosage and timing of dosage down, but once we did, the pay off is worth it. With that said, while figuring out what timing worked best we still saw some very immediate positive results. We found that splitting the dose and giving her 2 mg in the am and the second 1mg pill at 4:30 made all the difference with helping her sleep at night. I believe when we just gave her one time dosing of the 3mg in the morning, by bedtime it was pretty much out of her system which meant she was expecting inner hyperactivity and a return to non medicated state.
So….the Guanfacine was a total game changer and helped a great with her impulsivity, frustrating tolerance and rejection sensitivity. BUT has not eliminated her anxiety 😥
We’re currently trying to get that figured out. I’m told it is another issue that is a separate condition from the adhd.
