Has anyone had a positive donor specific test post transplant?

Hi, im new to this forum but could you explain the issue to do with a positive dsa test, post transplant? I had a living donar transplant in feb 2013 and ive not heard about a positive dsa test, post transplant.

Hi, firstly congrats on your transplant, hope you are well. I had my transplant 22 years ago. To he honest with you I dont know the answer. All I know is that I have had the test recently and it came back positive. I know it can cause rejection. I am having more bloods tomorrow and seeing my consultant Friday. I' ll let you know what info I find out.

Hi akodjoglover49, I have a little more info for you....although the first thing my consultant said was " The medical world are still trying to work out what to do with people like you!!!" The positive dsa means I am producing a particular antibody that is attacking my transplanted kidney. They are going to put me on an extra anti-rejection med and see if that helps.

Positive DSA is not an issue at all , the antibodies can be filtered out by using double filtration method and once they are out further formation of antibody can be prevented by using certain injection that throw out memory cells that produce thie DSA antibody

Hi iioss, thanks for your reply. Can you explain what double filtration is and what injection you refer to. After my DSA test I was put on extra wnti rejection drugs, however, these are not working as well as hoped. I had my transplant 22 years ago. EGFR 20 creatanine 220. I'm seeing my consultant next week so would be good to be armed with info. Thanks

yes there are many who develop what is called HLA and NoN HLA antibody to the graft that can lead to rise in creatinine, the usual procedure is to remove these antibody and then give rituximab to get rid of B cells that make these antibody. Also I give botronet which is anti plasma cell antibody. Then follow the titres of Anti HLA antibody , it is of less use if graft is already damaged and creatinine gone up
