is it normal to feel enlarged thyroid when extremely fatigued? I suffer from ME/CFS and have been struggling a lot recently due to stress, workload etc and lack of time to rest. When it gets real bad I feel strange sensation of tightness in the front neck, thyroid area. Almost love my thyroid is swollen or my neck muscles suddenly shrank.
has anyone ever had this?
is it possible for thyroid to reach in such way to extreme fatigue?

For me personally, my thyroid/throat absolutely felt like that, but it got better as I increased my Levo dosage towards optimal. My throat ached, my voice was easily tired out when talking or trying to sing along to the radio. My neck to the layman’s eye looks unremarkable, yet the day I was diagnosed my doctor could see slight puffy/swollenness from across the room. It took me a very long time staring at my neck to understand what he saw. Now I have to put my chin up to see the roundedness where my thyroid is. You can also read countless posts here about similar physical sensations/aches around our thyroid when hypo.
Have you ever had a thyroid ultrasound?
Did you ever pursue a Levo increase as discussed in your recent list?
As Tigger noted, and others shared, your bloods show you are under-replaced.
Hi, I kept thinking it’s just tiredness and stress giving me this sensation or anxiety. I had it in the past when my illness started and I had several ultrasounds due to the diagnose (Subacute Thyroiditis) and suspected cancer back then.
After the blue horizon 3 weeks ago I also had GP bloods for only TSH and T4 so difference in 3 weeks with no changes to my side. The reason why I did BH is because GP refused any more than those 2! And the results are:
TSH 2.85
Free T4 16.6
From two different labs I’m not surprised there’s a defence but 2.85 explains why I’m feeling unwell. I am still not sure that this is because irregular hours when I take my dose or too soon after food (taking at the bed time) or should I increase.
As you see, being in range, GP is very unlikely to change my dose or do any other tests. I left my BH results with them as there’s a suggestion I need 24h cortisol test. I’ve got an apt with them on 18/11 after holidays.
Also a question came to my mind, what test should I have for adrenal fatigue?
I too investigated cortisol issues, many of us here do a multi-point saliva test.
I’m in the US, so have no idea the hurdles in the UK to get one.
But I will also say for myself, I just needed to spend more time optimizing my thyroid hormones. My body had indeed compensated for years while undiagnosed with adrenaline and cortisol… and starting thyroid hormone replacement also threw my adrenaline/cortisol reactions into a mess for a while. But I’m settling down now on a near-optimal level of t4 and t3.
Searching cortisol on this board will give you a good idea about some common experiences with saliva tests. Anecdotally, cortisol issues often a suspect but sub-optimal thyroid hormones can sometimes (but always) be the root cause as well.
100mcg daily will be too high I believe. I use to be on 100 once a week and 75 the rest and that really messed me up and I vent way over with severe symptoms. I don’t want to go back there ever again.
I weigh 65kg which is way too much for 160cm high. Before the illness I was 50kg ☹️
