T3 Progress - next steps?

Since my last post healthunlocked.com/thyroidu... I have started taking Thybon Henning to optimise my T3 with some success. Currently taking 10 mgs T3 at night, and have dropped my levo from 125 to 100. T3 has gone to 4.7 from 4 but I understand it should be nearer to 6. However, TSH has increased from .885 to 1.72 in a year, and T4 has dropped from 21.5 to 13.9 since taking T3.

Questions

- I am thinking that I will increase Thybon to 15mcg ?

- Should i be worried about my T4 reducing

- No idea if related but I had covid in May I am recovering from, and also have developed oral thrush so clearly something else going own in the microbiome.

Results and trends attached. As always very grateful for any inputs.

Can I just clarify that the ‘100 levo’ result above is really T4 100/T3 50 and you’ve been on that new dose how long assuming you very slowly introduced T3? It does look like a huge dose for your blood results…to me it would suggest perhaps the oral thrush ( and treatment?) is making a difference to absorption. FT4 does go down with T3 in my experience. I have had Long Covid for 4 1/2 years and in my case it has ‘knocked on’ effects to lots of my body…

oops sorry yes, I went through Roseway started on 5mcg for 6 weeks and been on 10mcg for the last few months.

I sympathise with you on Covid. Took me a long time to recover 3 yrs ago and since having it since May has knocked my fitness which was good (40 vo2 max) to 28 (poor). Still learning I think it affects so much.

‘10 mcg for last few months’…how does 50 mcg T3 written in original post fit in?

It doesn't its just my ADHD brain getting mixed up. Apart from the introduction in May its been 10mcg

Got it, so yes touch more ie 2.5 -5 more T3, you could expt. adding it to other doses or put a 3rd separate dose in mid afternoon.

That’s a very high dose of T3

When did you start on T3

We recommend starting SLOWLY

Usually adding 5mcg ONLY to start

And only increase by 5mcg at any one time

Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

T3 ….day before test split T3 as 2 or 3 smaller doses spread through the day, with last dose approximately 8-12 hours before test

Is this how you did your test

If you left too long between last dose T3 and test result will be false low

Your low Ft4 is because you reduced your levothyroxine

Many people find they need BOTH Ft4 and Ft3 at good levels

Suggest initially you INCREASE Levo back up by 12.5mcg daily to 112.5mcg daily

Retest in 8 weeks

Meanwhile when were vitamin D, folate, ferritin and B12 last tested

Sorry started T3 on 1/5/24 on 5mcg then to 10 after 6 weeks which is what I am now. Tested last week (in original post) I took all my T3 12 hours before and the levo 24 hours before. 9am test after fasting and just water.

Also I am taking vit D+K, B complex, Magnesium, Zinc,Fish Oil, L-Glutamine.

Should I increas the Thybon?

So you are currently only taking 10mcg of T3 daily

Is that correct?

(Not 50mcg T3 per day as you said in opening post above)

Did you stop vitamin B complex 5-7 days before test as it contains biotin

Yes thats correct. I stopped all vitamins 24hrs before, I didnt appreciate that the Bs have Biotin

Ok

So I have just edited your opening post - text in bold

Currently taking 10mcg T3 daily

Considering increasing to 15mcg T3 daily

You could either

A) increase T3 by 5mcg to 15mcg daily - suggest you split the dose, certainly initially

Retest in 6-8 weeks

Then AFTER next test ….likely to want to increase levothyroxine to improve low Ft4

Or

B) increase levothyroxine initially to 112.5mcg daily

Retest in 6-8 weeks. See if Ft4 has stopped dropping/started to improve

Likely to need to increase T3 by 5mcg AFTER next test to improve low Ft3

NEVER increase both levothyroxine and T3 …..only change one thing at a time

Up to you which you try first

I was working on the basis that getting the T3 right is the most important, but from what you are saying T4 needs to be correct as well. And I shouldnt worry about TSH if its around 1 although my GP gets excited if its below that (no not meaning to get you started :-) )

Adding T3 almost always significantly reduces or suppresses TSH

TSH will frequently drop well below range

Most important results are always Ft3 and Ft4

GP shouldn’t meddle.

Looking at previous posts you are prescribed T3 via thyroid specialist

Presumably this is on private prescription?

What, if any, other symptoms do you have?

T3 will naturally lower both TSH and FT4 so don't worry about lower FT4

Both Frees should be roughly approaching 75% through their respective ranges.....for example

Your FT3 on 09/24

4.7 ( 3.1 -6.8) is 43.24% through ref range

For most this would be too low, so increase advisable!

TSH is not a reliable marker....important labs are FT3 followed by FT4

Important to optimise vit D, vit B12, folate and ferritin to support thyroid function

Only change one thyroid hormone dose at a time.

In your shoes I would add another 5mcg T3 following advised protocol and re-test in 6 weks.

Morning dippydame and thank you for your message. Rather annoyingly I have posted 2 long replies to you yesterday, neither of which seems to have been posted, so I will try again!

I haven’t seen the list of symptoms, but it comes as no surprise that I have or have had probably 2/3rds of them – I’ll try posting the image separately. The fact that there is a linkage with ADHD, thyroid, gluten/food intolerance and my IBS symptoms has been illuminating. Like you and others I am on a constant journey to get to the bottom of things.

With regards to the oral thrush, I am awaiting anti-fungal prescription. However, gargling with ACV and salt water, as well as reducing dairy and sugar seems to have made an effect in the couple of days I have been doing this. Unfortunately, as my lungs have been compromised by my recent COVID infection, I need to take some steroids which will aggravate the situation.

As has been suggested I will increase my Thybon by 5mcg and test again in a couple of months.

Your bio is an epic read- thank you for posting. Like you I had a serious RTA at 18 and have had 2 major concussions. The last one 5 years ago has taken along time to recover and I only started making progress a couple of years ago when I discovered my hypothyroidism and ADHD. Your contribution and others have made this group a lifeline in treating my condition.

Hello Autumn welcome you have come to a good place. Yes you need a prescription in the UK but it's possible to buy abroad without one. Roseway labs have been used a lot here and they offer a consultation and prescription service in the UK. I found them to be very good.
