newly diagnosed Hyperthyroid

hello, I am new to this forum and would love some advice/further insight on Hyperthyroid. I had a private hormone blood test which has come back with an out of range result for Thyroid function

TSH 0.006 Range 0.270-4.2

T4 40.6 Range 12-22

My GP will be doing a referral to Endo and I am booked in for more bloods, didn’t really say anything anything else other than I have a hyperactive thyroid, do you think the results are anything to worry about?

How are you feeling? Do you have some symptoms which prompted you to arrange a blood test?

You’ll need to have a much more complete blood test. On the next test see of Dr will test.

TSH

FT4

FT3. FT3 is the active hormone & most symptoms ar from this measure although doctors tend to focus on TSH.

Antibodies should also be tested, to see if you have an autoimmune condition.

1 can cause thyroid levels to fluctuate initially and gradually your thyroid under function & replacement is needed, (Hashimoto’s also called autoimmune thyroiditis) another continuously stimulates thyroid (Graves) and you’ll need anti thyroid medication

TPO & TGab are present with both autoimmune,

TSI or TRab if Graves suspected.

Also important to test

B12

FERRITIN

FOLATE

VITAMIN D

Most of the feedback we seem to get is that GPs can request TPO antibodies to be tested if hypothyroidism is suspected but only a hospital specialist can order a TSI or TRAb is Graves if suspected with low TSH & high FT4 & FT3.

If FT4 is lower on next test it might be a sign levels will naturally settle. You still need further monitoring.

If levels are still high you’ll likely be started on an antithyroid medication to lower production of new hormone. Usually this is carbimazole or PTU. Propranolol, as beta blocker is also sometimes offered as it helps relieve common symptoms especially heart palpitations. (Not if asthmatic).

hello! Thank you for responding and apologies for my delayed response, it was actually picked up from a private well women blood test that i got through work.

I have had further blood tests from my gp.

Serum TSH 0.02 Range 0.3-4.5

FT4 36.4 Range 11-22

FT3 9.7 Range 3.1-6.8

Antibody TSH <1.0 Range 1.0-1.0

I get occasional palpations and do suffer from anxiety but other than that I don’t really think I have anything else. X

They are quite high levels.

The Antibody TSH <1.0 Range 1.0-1.0. This may be TRab TSH receptor antibody. I’ve never seen a range as (1.0 - 1.0) they are usually (0 - 1)

You may have borderline result. Worth test TPO TG antibodies too.

Has GP suggested any medication while you are waiting for endo appointment?

Sorry, that range should be 1.0-1.8, i did have a test for TPO but the results are blank.

I havent had any medication prescribed, its been radio silence from my doctor, the reception said it looks like a referal has been made but i havent heard anything yet. I dont know what to think about it really as not hearing anything makes me thinks its nothing to really worry about, but then from reading forums mades me wonder if i should be worried

Welcome to the forum. It’s mostly underactive people on here but you are overactive. Do you have any symptoms ? Any swelling in your neck or nodules? Why was your thyroid tested?

Do you supplement with iodine by any chance?

Your GP should have given you a bit more information while you wait for an Endo appointment. Have they given you a time scale ?

There are lots of things that could cause high T4, some could be just transient while other just need medication or some even surgery. I think you should seek some more info from your GP while you wait for your endo appointment. Were any other bloods out of range?

Hello Amme and welcome to the forum :

What were the symptoms that took you to the doctor and has your doctor prescribed anything while we wait for an appointment with a hospital endocrinologist ?

This T4 looks a bit high - the range you have detailed has a cut off of 22.00 do you also have there a T3 blood test result and range ?

I would imagine if a referral isn't forthcoming quickly your doctor will be instructed to monitor you and take instruction from the hospital endo team.

There is more than one reason for your thyroid to have become over active - if there is no obvious sign of a swelling in your neck and you feel no constriction when swallowing or breathing there are a couple of Auto Immune health issues that can attack the thyroid and cause outside the range thyroid levels.

In order to know which AI disease you maybe dealing with you do need thyroid antibody blood test - and we are looking for either TPO or TgAB for Hashimoto's AI disease and / or TRab or TSH Thyroid Receptor antibodies for Graves Disease.

Is your heart involved - are you aware your body is ' running faster ' than normal ?

Are you taking any other medications or supplements ?

hyperactive thyroid

Just a note on terminology :

a) You can be hyperthyroid

or

b) You can have an overactive thyroid

but you can't be hyperactive thyroid.
