I have just received these iron blood test results, could someone please look over them for me as I’m unsure if I should be supplementing or not?

What are your most recent thyroid and vitamin D, folate and B12 results

Are you hypothyroid and on replacement thyroid hormones

is your hypothyroidism autoimmune

Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

If on T3 ….day before test split T3 as 2 or 3 smaller doses spread through the day, with last dose approximately 8-12 hours before test

Private tests are available as NHS currently rarely tests Ft3 or all relevant vitamins

I have attached here my recent bloods, I’m now on b12 injections. I also take folate and vitamin d.

Which brand

How long on this dose

Free T4 (fT4) 18 pmol/L (12 - 22) 60.0%

Free T3 (fT3) 4.2 pmol/L (3.1 - 6.8) 29.7%

Was test early morning and last dose levothyroxine 24 hours before test

Assuming yes…..shows poor conversion of Ft4 to Ft3

B12 still low

How frequently are B12 injections

Rather than just taking folate

Then once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to reduce then stop the B12 and just carry on with the B Complex.

If Vegetarian or vegan likely to need ongoing separate B12 few times a week

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule) Thorne can be difficult to find at reasonable price, should be around £20-£25. iherb.com often have in stock. Or try ebay

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg) and continue separate B12

Thyroid peroxidase antibodies are 466

I had my thyroxine increased by 25mg after this test so I think that’s why the results aren’t the best. My FT3 usually is at a good level when I was on the higher dose when pregnant.

Only recently just started having more b12 injections, I’m now having them daily until my symptoms improve and then going to weekly.

I take methyl folate already but I’ve increased them.

Also can’t absorb b12 tablets so that’s why I’ll be staying on injections.

So you probably still would benefit from vitamin B complex rather than just methyl folate

Have you had coeliac blood test

Are you on gluten free/dairy free diet

No not had coeliac bloods, doctor won’t do them as not showing symptoms. Also no not gluten or dairy free this would be extremely difficult for me.

Hashimoto's affects the gut and leads to low stomach acid and then low vitamin levels

Low vitamin levels affect Thyroid hormone working

Poor gut function can lead leaky gut (literally holes in gut wall) this can cause food intolerances.

Most common by far is gluten.

Dairy is second most common.

A trial of strictly gluten free diet is always worth doing

Only 5% of Hashimoto’s patients test positive for coeliac but a further 81% of Hashimoto’s patients who try gluten free diet find noticeable or significant improvement or find it’s essential

A strictly gluten free diet helps or is essential due to gluten intolerance (no test available) or due to leaky gut and gluten causing molecular mimicry (see Amy Myers link)

Changing to a strictly gluten free diet may help reduce symptoms, help gut heal and may slowly lower TPO antibodies

While still eating high gluten diet GP should test for coeliac blood test first as per NICE Guidelines

Assuming test is negative you can immediately go on strictly gluten free diet 

(If test is positive you will need to remain on high gluten diet until endoscopy, maximum 6 weeks wait officially) 

Trying gluten free diet for 3-6 months.

If no noticeable improvement then reintroduce gluten and see if symptoms get worse

Similarly few months later consider trying dairy free too. Approx 50-60% find dairy free beneficial
