T3 for treatment resistant depression

My son is 30. He has been suffering with a major depressive disorder for over 10 years. In that time, he has been prescribed every anti depressant under the sun, including Lithium, none of which have worked. He was sectioned last summer having been considered a danger to his own life. He is also a complete insomniac, which exacerbates the depression. After 6 long months of appointments, an NHS psychiatrist finally asked his GP to prescribe T3 for him, even though he is Euthyroid, as it has apparently been know for many decades that T3 has a very high success rate in treating treatment resistant depression. It begs the question why this has not been suggested sooner, but that is another story. Unbelievably in the circumstances, the GP has flatly refused to prescribe T3. There seems to be a blanket ban on its prescription in certain health authorities, and certainly off label, as this would be, despite having been requested by the psychiatrist! I should stipulate that even though my son's Thyroid test results fall within the "normal" range, his T4 is usually about 14 and has sometimes been as low as 12, so there is a lot of room for improvement. I should add that there is a lot of hypothyroidism in the family- I have been taking thyroxine since 1993 and two of my sisters also do. So what I would like to ask is whether anyone has any expertise/experience with this or knowledge they could share. In particular, does anyone know if we would be able to source T3 at a reasonable price without a prescription (I am not sure at this point whether the psychiatrist can actually provide a prescription or whether that is in the remit of the GP). Many thanks.

I don't live in the UK, but my understanding is that the GP cannot initiate prescribing T3 him/herself, it has to be an endo. But, the psy can. And I'm pretty certain some do.

Question is: does the psy know anything about treating with T3? And, if you managed to buy on-line without a prescription, will he survey the treatment?

I would strongly recommend you get him FULL thyroid and vitamin testing asap

For full Thyroid evaluation he needs TSH, FT4 and FT3 tested

Also both TPO and TG thyroid antibodies tested at least once

Very important to test vitamin D, folate, ferritin and B12 at least once year minimum

About 90% of primary hypothyroidism is autoimmune thyroid disease, usually diagnosed by high thyroid antibodies

Autoimmune thyroid disease with goitre is Hashimoto’s

Autoimmune thyroid disease without goitre is Ord’s thyroiditis.

Both are autoimmune and generally called Hashimoto’s.

Low vitamin levels are extremely common when hypothyroid, especially with autoimmune thyroid disease

20% of autoimmune thyroid patients never have high thyroid antibodies and ultrasound scan of thyroid can get diagnosis

In U.K. medics hardly ever refer to autoimmune thyroid disease as Hashimoto’s (or Ord’s thyroiditis)

Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Private tests are available as NHS currently rarely tests Ft3 or all relevant vitamins

Testing options and includes money off codes for private testing

Medichecks and BH also offer private blood draw at clinic near you, or private nurse to your own home…..for an extra fee

Sounds likely he already has autoimmune thyroid disease

If both antibodies are negative get ultrasound scan of thyroid organised privately

20% of Hashimoto's patients never have raised antibodies

Gluten intolerance is often a hidden issue too.

Request coeliac blood test BEFORE considering trial on strictly gluten free diet

But even if he hasn’t got autoimmune thyroid disease Psychiatrist can /should prescribe liothyronine trial directly for at least 3-6 months

Where liothyronine is prescribed for the treatment of depression only, this should be under the advice of an NHS consultant psychiatrist.

Where liothyronine is prescribed for the treatment of depression in patients with suspected or established thyroid disease, this should be under the advice of an NHS consultant psychiatrist and NHS consultant endocrinologist.
