Hi there I’m almost optimally treated now with levothyroxine but still don’t feel 100%.. The gym used to energise me now I just feel exhausted the day after..I’ve radically changed my diet and I don’t drink alcohol very much either. My brain fog is a lot better but energy and libido still not what it was 4-5 years ago. I wonder whether some of the natural products in metavive May give me the boost I need? I’m now taking 87.5mcg Levo but still feel the same.

What makes you think you're nearly optimally treated now? If you still have all those symptoms then you aren't near optimal. Optimal is how you feel, not a blood test result.
Feeling exhausted the day after going to the gym probably means you shouldn't be going to the gym. Your FT3 just isn't high enough to support it. Just do gentle walking or swimming until you get your FT3 higher.

What natural products? It's not a boost you need, it's an increase in dose. 87.5 mcg is still only a small dose. And if you don't feel any better despite the increase, then it's still not enough.
Metavive suits some people but not others. I've never tried it myself so I can't speak from experience.
Thanks greygoose I didn’t think I had room for an increase to 100mcg my T3 is currently 54% through range and T4 at 60% TSH is 0.35.. that was on 75mcg.. I was thinking metavive has T1 and T2 etc 🤷♀️ I hear lots of people on here saying that they had so much more energy switching over to glandular or NDT. Is that just because their T3 is higher?
I think it would take more than 100 mcg to push your FT3 over-range. And, besides, some people just need it over-range. Ranges are just rough guides, not cast-iron rules.
The question of T1 and T2 are sticky ones. No-one has even been able to answer the question: does the thyroid actually make any T1 and T2 or are they just by products of iodine recycling? If the thyroid does make any, it would be such a teeny, weeny, tiny amount that it would be insignificant. This argument goes on and on over NDT. A lot of people don't believe NDT contains T1 or T2, and even if it did, would they be of any help? Your body is making T1 and T2 all the time - T4 to T3, T3 to T2, T2 to T1, T1 to T0 - it's doubtful that you would be low in it. So, if Metavive helps people, I doubt that's down to the T1 or T2 it may or may not contain.
Have you had your testosterone levels checked? My hormones all crashed after years of untreated hypo... I now add them all back in but testosterone is the resilience and libido giver
I'm with GG that you could do with a little more T4 perhaps 12.5mcg every other day?
Thanks Eeyore100 yes at the moment I’m taking 75mcg one day and 100mcg the next so 87.5mcg..do you think I could go to 100mcg or would that be too much? I’ve had my Testosterone levels checked and my free testosterone was 0.26 (0.16-0.47) so not particularly great to be honest. It’s strange because my libido got worse when I started the levothyroxine. Actually the first 5 days on Levo the libido came back but then went and still hasn’t come back. It was better when I was hypo and on nothing, how does that work 🤷♀️. I do feel better in myself brain fog anxiety zest for life is coming back etc but still lacking in energy and libido.
Maybe 5 days on 100 and 2 on 75... when you are hopefully nearing your sweet spot adding in small amounts is the only way to fine tune... chances are with winter coming going up to 100 might be where you end up... strangely if you take your fT4 too high it often ends up with less converting to fT3 so we want to avoid that
All your other factors are at a good level for conversion so hopefully just tweaking your T4 up gradually you'll get that little uplift that makes all the difference 🤗
You could try for 2 weeks gauge how you feel and then go to 6 days at 100 etc
It takes 6-8 weeks or more to settle but you generally get a feel for it within 2
As GG says pacing yourself in the gym is going to help as overtraining really knocks you back
as a bloke you are not on very high dose levothyroxine

When did you last test vitamin D, folate, ferritin and B12

These look too low
So retest after 6-8 weeks on 75mcg and 100mcg alternate days
Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test
This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Thanks SlowDragon yes I’m 93 kg so a decent size. All my vitamins are good tested 3 weeks ago and posted here. I’ll retest again in 3 weeks and then see where I am as to whether I move up to 100mcg. Thanks again.
