Pregnancy thyroid checks

Hi all,

I just wanted some advice on thyroid management in pregnancy. I have 3 kids but wasnt treated with hypothyroidism then. I had a miscarriage in 2016 and then another early this year. I am now 14+4. Was tested in July when i was cca 4 weeks and my results were: TSH 0.99 and FT14.2. This was before a dose increase. I was om 75mcg Euthryox which I source from the Czech rep and because i didnt want tp change brands I now take 75 5 days and 150 (so 2 tablets) 2 days.

GP said they she would refer me to Thyroid clinic at Obstetrics. I havent heard from anyone since. I had an endo appt during the summer in Czech rep - my results on 15th august were TSH 0.514

(0.380-3.500) and ft4 14.30 (9.01-19.05). Endo was happy and said i needed to be tested again in 4-6 weeks.

Midwife at booking appt said she cant action a TSH test and that thyroid levels are more important later in the pregnancy anyway (?!). So I had a letter through from Obstretics for a phone consultation on 29th September - so basically 12 weeks after my blood test in July.

Am I right in thinking that there should have def been more blood tests? I didnt tell midwife about my results in Czech rep and reminded her that noone here tested me after dose change but she kept saying she cant action it and that Obstretics will deal with it. Are they relying on me to arrange tests or am I right in thinking that there should have been more follow up given the hypothyroidism and my history with miscarriages?

I’m sorry you are going through this. I am afraid the experience people have with pregnancies vary by areas and even GP/Midwifery services during pregnancy. With that said, there is nothing to stop you requesting more frequent testing.

Be aware of trimester specific ranges for TSH and thyroid in pregnancy which do vary according to area.

As far as I can tell, there is a disconnect between specialist support from endocrinology obstetrician (usually actioned at booking appointment) and it is expected that you are looked after by the GP before then. Your first step is to contact the GP explaining that you’ve not been tested at beginning of pregnancy or since change in dose which I believe is quite standard practice.
