Yellow Card Reports

The Yellow Card system is the main way for reporting issues with medicines.

Reasons to consider making a Yellow Card report:

• If you have a side effect or adverse event.

• If a medicine seems different to the last batch.

• If one make of a medicine seems different to another make of the same medicine.

• If the medicine has physical problems such as crumbling, or not having the Patient Information Leaflet in the pack.

Most importantly, professional medical staff relatively rarely make Yellow Card reports. It really is down to each of us to put in reports, if we see a reason to do so.

You don’t have to prove anything. Simply thinking that there is an issue is sufficient.
