Thyroid blood tests normal

hi, I have been having hypothyroid symptoms for some time. Had bloods done….could someone give me any advice as they are within normal range apart from Vitamin D., which was 41. Thanks

Do you have a diagnosed thyroid condition, currently take any replacement, or suspect you do?

What time was test taken?

TSH & FT4 isn’t a complete thyroid test. Have you ever had FT3 or thyroid antibodies tested?

Are you taking a vitamin D supplement or any other supplements such as one with biotin in? Biotin can affect blood test results.

Hi, thank you.

I do not have a diagnosed thyroid condition. I just suspect that there’s a problem with thyroid. The test was done at 09:50.

They didn’t do a FT3 or antibody test.

I am now taking Vitamin D but not before the test. I don’t believe that I’m taking anything with biotin in.

As the results are in range doctors will not test further.  

Doctors do not look to start replacement unless TSH is over 10. Or FT4 below range.  

If TSH is over range but below 10 the test is repeated in 3 months.

The TSH can be unreliable and your FT4 is low so if might be worth testing FT3 the active powerful thyroid hormone and thyroid antibodies. 

There are private options as NHS do not complete complete test.  You order test online the kits arrives via post sample taken by finger prick (extra fee for private venous draw). Post back and results available online quite quickly.

When having a thyroid blood test - Arrange draw close to 09.00 as possible.  Cease biotin days before draw to avoid possible test interference.  Fast overnight.  This can have a slight impact on TSH.  Drink lots of water.

Was test done early morning?

Ft4 is low

Vitamin D is insufficient

What has GP prescribed

Any results for folate, ferritin or B12?

Suggest you request new thyroid test in 6 weeks - request thyroid antibodies tested as well as TSH, Ft4 and Ft3

Book early morning 9am test

For full Thyroid evaluation you need TSH, FT4 and FT3 tested 

Also both TPO and TG thyroid antibodies tested at least once 

Very important to test vitamin D, folate, ferritin and B12 at least once year minimum

About 90% of primary hypothyroidism is autoimmune thyroid disease, usually diagnosed by high thyroid antibodies 

Autoimmune thyroid disease with goitre is Hashimoto’s

Autoimmune thyroid disease without goitre is Ord’s thyroiditis. 

Both are autoimmune and generally called Hashimoto’s.

Low vitamin levels are extremely common when hypothyroid, especially with autoimmune thyroid disease (Hashimoto’s or Ord’s thyroiditis)

20% of autoimmune thyroid patients never have high thyroid antibodies and ultrasound scan of thyroid can get diagnosis 

In U.K. medics hardly ever refer to autoimmune thyroid disease as Hashimoto’s (or Ord’s thyroiditis)

Recommended that all thyroid blood tests early morning, ideally just before 9am (and last dose levothyroxine 24 hours before test )

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Private tests are available as NHS currently rarely tests Ft3 or all relevant vitamins
