EXHAUSTION & Help with Labs

I just got blood results back after feeling completely EXHAUSTED for quite some time (sleeping 9-12 hours a night and feeling tired most of the day). I currently take 2.5mcg of T3 and 6am and 2pm and 88mcg of t4 Tirosint at 10pm. Should I decrease my t3 meds? Please help! TIA

TSH (0.40-4.50 ): 0.22 LOW 

T3 Uptake (22-35 %): 37 HIGH 

T4 Total (5.1-11.9mcg/dL): 6.2 

Free T4 Index (1.4-3.8 mIU/L): 2.3 

T3, Free (2.3-4.2 ng/dL): 3.9

Did you test early morning

Was last dose levothyroxine 24 hours before test

If you normally take levothyroxine at bedtime/in night ...adjust timings as follows prior to blood test

If testing Monday morning, delay Saturday evening dose levothyroxine until Sunday morning. Delay Sunday evening dose levothyroxine until after blood test on Monday morning. Take Monday evening dose levothyroxine as per normal

And last dose T3 should be 8-12 hours before test

When were vitamin D, folate, B12 and ferritin levels tested

I followed the directions from my doctor and she said to take meds as normal. Just thyroid panel was done. Simply seeking feedback based on the results.

I see you joined the Forum in 2019 so you will have read many times the patient to patient advice about the timing of meds prior to testing. Also the importance of optimal levels of B12 - Folate - Ferritin - VitD so your thyroid hormones can work well. I appreciate you stated you only wanted comments on the results above .... sometimes the bigger picture is required in order to comment helpfully 🌻

What time was test done

so if you took levothyroxine at 10pm and tested 9am Ft4 would be falsely high as last dose only 11 hours before test

Ft3 falsely low if last dose T3 more than 12 hours before test

Free T4 (fT4) 2.3 pmol/L (1.4 - 3.8) 37.5%

Free T3 (fT3) 3.9 pmol/L (2.3 - 4.2) 84.2%

If you had taken last dose levothyroxine 24 hours before test Ft4 result would have been lower

If last dose T3 was 8-12 hours before test result would have been higher

So results suggest you need more levothyroxine

And less T3

Vitamin levels need considering too

I am on similar but 5mcg of t3 and tirisont 100mcg. I feel exactly the same. I am so exhausted and getting alot of sleep some nights. I have fatigue all day but one of the worst symptoms for me is the all over body aches and pain. It started as soon as I pretty much started tirosint and most days is horrendous.
