Help need advice required Dr is going to decrease my T4 when i think he needs to add T3!

Taking 6 ml of levo oral solution per day.

have many of hypothyroid symptoms including terrible brain fog, wake up 6 times a night, always cold the list goes on. I think i also have low cortisol

I believe he should add a T3 treatment or he or I should source an NDT.

THIS IS WHAT MY DR SENT TODAY

Your Free T3 level is 3.9 pmol/L (normal range 3.1 to 6.8) - NORMAL

Your Free T4 is 19.5 pmol/L (normal range 11.3 to 21.6) - NORMAL

Your TSH level is 0.19 mu/L (normal range 0.27 to 4.20) - SUPPRESSED ie body's feedback is detecting too much circulating thyroid hormone and so suppresses TSH production.

Can someone please advise If my Dr is right about his diagnosis to lower my T4.

No, but that's what they're taught. He's only looking at TSH and ignoring the fact that your FT4 is in range and your FT3 is close to the bottom of the range.

Your results clearly show poor conversion of T4 to T3 and I doubt that your doctor even understands that.

What are your key nutrients like? These need to be optimal for thyroid hormone to work and good conversion to take place, so have you had the following tested:

Vit D

B12

Folate

Ferritin

If these are all optimal then you should be referred to an endo for a trial of T3.

A GP in the UK cannot initiate T3 treatment. This has to be done by an endo for a trial at least 3 months in the first instance.

NDT is not licensed for use in the UK. Any doctor prescribing it has to take personal responsibility for its use - so it is rare to get a prescription for it.

Are you splitting your liquid levothyroxine into two smaller doses per day

Was test done early morning, ideally before 9am and last dose levothyroxine 24 hours before test

Results show poor conversion

Next step is to test vitamin D, folate, ferritin and B12

What vitamin supplements are you currently taking

Do you have autoimmune thyroid disease, also called Hashimoto’s…usually diagnosed by high thyroid antibodies and if so are you on strictly gluten free diet or dairy free diet

Hi Slowdragon thank you so much for getting back to me.

i take the 6ml in the morning so will try splitting it . I am currently taking vitamin D3 with k2, selenium, omega 3, vitamin B complex , zinc with copper.

blood tests were done at 0850 and i had not taken my levo for 24 hours . I have Hashimotos diagnosed 20 plus years ago, and follow the AIP protocol so strictly no gluten or dairy .

Dr did my bloods although not folate or vitamin d as lab forgot to do them due to a mix up .

These are the results i have so far

Serum iron 26.2 (range 5.8- 34.5)

Serum transferrin 2.6 ( range 2.00-3.60)

Transferrin saturation 24.8 (range 15-45

Serum magnesium 0.8 ( range 0.7-1.00)

Serum zinc 15.6 (range 10-22)

Serum vitamin B12 518 (range 197-771)

Serum iron 16.2 (range 5.8-34.5)

Serum transferrin 2.6 (range 2-3.6)

Transferrin saturation 24.8 (range 25-45)

I will ask him to arrange to do vitamin D and folate tests again if you could let me know what the optimal ranges are supposed to be.

vitamin D at least around 80nmol and around 100nmol maybe better

Suggest you add a magnesium supplement in afternoon or early evening at least 4 hours away from levothyroxine
