First of all we need to know if you took your test as we advise:
* No later than 9am
* Water only before the test so that no food or drink interferes with TSH level]
* Last dose of Levo 24 hours before test to avoid a false high or false low result
* No biotin or B Complex or any supplement containing biotin for 3-7 days before the test.
If so then your results are as accurate as can be and can be interpreted as follows.
Your current results show that you are very hypothyroid to have such a high TSH and such a low FT4.
Why was your Levo reduced from 150mcg to 100mcg, and was it reduced in one go or 25mcg at a time?
Your thyroid peroxidase antibodies are elevated which confirms autoimmune thyroid disease, known to patients as Hashimoto's. This causes swings from stable to hypo and false hyper episodes. It looks like you are in a hypo episode at the moment and possibley you were in a false hyper episode when your Levo was reduced.
Past posts show that you were on a combination of Levo and T3, then Thyrogold and then NDT. Now you seem to be back on Levo which at one point you said was making you really poorly. How come you're back on Levo?

The reason I am back on Levo is because my endocrinologist point blankly refused to give me a consultation or blood tests unless I was taking the prescribed meditation - which originally was 175mcg levothyroxine. (she doesn’t trust NDT!) Yes, I had been taking Thyro-Gold for quite some time then changed to Thyroid S when I couldn’t source Thyro-Gold during covid. Apparently, I had made myself severely ‘hyperthyroid’. I agreed to try the levo again ‘just to get an idea of my blood results. For some reason my GP prescribed 150mcg instead, then without warning in March the chemist only gave me 100mcg saying to speak to my GP. Apparently my TSH was severely suppressed. The levo makes me feel very poor so I have started taking Thyroid-S again (one 65mg tablet = 100mcg levo?). I only planned to take the Levo to get the appointment and blood tests as I was really suffering and feeling so depressed. I was desperate to get help.
The latest blood test was taken at 11am in a fasted state (no food or drink other than water since 8pm the night before) and no biotin or B vits.
I really appreciate your help. Since all this messing about with meds since December I have gained 3 stone of the 6 I lost when I was stable on Thyro-Gold. I am fasting 18-6 and having one healthy low carb/Keto meal a day so I’m certainly not overeating OR eating the wrong things.

Presumably a GP test rather than endo? Whoever made that comment then make an appointment to discuss and ask why such obviously abnormal results do not require adjusting dose of Levo.
If you were so much better on Thyro Gold or NDT then why not go back on it and do your own tests to monitor your levels.
I have started it again yesterday, but I’m still unsure of the correct dose. I read 1 tablet Thyroid S (65mg) is equivalent to 100mcg levo.I wish I could show you the letter the endo wrote to my GP, then you would understand why I felt bullied back in levo.
During covid I purchased several Medichecks and they ended up part refunding for the T3 as I cannot draw enough blood from my fingers. It tends to thicken immediately after a single drop no matter how I warm my finger and rub it.
So, I’m having to rely on having the blood taken. Additionally I have chronic Lyme disease (from the age of 5 years) so getting my thyroid results is really important due to my immune system issues.
What a nightmare for you Bluehaven. Its a shame you felt bullied into going back on levo. That would be a big red flag for me re your Endo. Have you thought about getting a second opinion? If you go for this please ensure you research carefully who you are referred to.....many endos know little about hypothyroidism being diabetes specialists.
This result above shows you are very under medicated
TSH should always be under 2 when on levothyroxine and Ft4 at least 60-70% through range
If you were staying on just levothyroxine, you would increase dose to 125mcg ….wait 6-8 weeks….retest
Likely to need to be on at least 150mcg

Suggest you pay for private blood draw and get FULL thyroid and vitamin testing done via Medichecks or Blue horizon 6-8 weeks after back on NDT
Always test thyroid levels early morning
Day before test split NDT into 2 or 3 smaller doses spread through the day and last dose NDT approx 8-12 hours before test
