Letter rather than a phone call about a lesion found

I'm in a bit of a state of bad anxiety about a letter from my endo doc. It came 2 weeks after a thyroid ultrasound and stated they had found a lesion posterior to the left lobe and would be referring me for a SPECT scan. Its now been a week since I received the letter and still not heard anything

I have no idea if the silence means it's not a big deal or I should be worried! The C word is always going to be in the back of my mind the moment you read the word "lesion"

He did use the words "suggestive of a parathyroid adenoma" but no idea if this means there is something on the ultrasound that pointed to this or that it is just something that it could be. My calcium levels when last checked were in the normal range however my Vitamin D was low. My GP is also dragging his heals in prescribing me Vit D replacement therapy

How low was your Vit D level, this question was asked by SlowDragon in response to a previous post a few days ago but you didn't reply.

If you have Vit D deficiency where level is below 25nmol/L then your GP should be following the NICE Clinical Knowledge Summary and provide loading doses of D3 and there is no reason why you would not have been given a prescription immediately upon receipt of those results.

If your level is between 25 and 50nmol/L your GP MAY prescribe but also may not.

If your level is over 50 then GP is unlikely to prescribe.

It is very easy and cheap to provide your own Vit D, which a lot of GPs suggest if Vit D deficiency is not present.

If you post the result we will be able to comment further.

I think my Vit D level was around 17.8

So if that's 17.8nmol/L then contact your GP and ask him to follow NICE treatment summary for Vit D deficiency: cks.nice.org.uk/vitamin-d-d...

(click on Management > Scenario:Management)

"Treat for vitamin D deficiency if serum 25-hydroxyvitamin D (25[OH]D) levels are less than 25 nmol/L.

For the treatment of vitamin D deficiency, the recommended treatment is based on fixed loading doses of vitamin D (up to a total of about 300,000 international units [IU]) given either as weekly or daily split doses, followed by lifelong maintenance treatment of about 800 IU a day. Higher doses of up to 2000 IU a day, occasionally up to 4000 IU a day, may be used for certain groups of people, for example those with malabsorption disorders.

* Several treatment regimens are available, including 50,000 IU once a week for 6 weeks (300,000 IU in total), 20,000 IU twice a week for 7 weeks (280,000 IU in total), or 4000 IU daily for 10 weeks (280,000 IU in total)."

If it's 17.8ng/ml then it's not deficiency but a little bit low.

Come back and tell us what the GP says. There is more to treating Vit D deficiency than just D3 so I will let you know what else to do when I know what your GP is doing.

Just rang my GP and they said that my normal doc has seen the letter but didn't prescribe it. And the receptionist said they are just waiting to see why he didn't when he's back in tomorrow

Actually I remembered wrong, it was 19.4

Still in the deficiency category so you need the loading doses. Sloppy care from the doctor if he read the letter and didn't prescribe!

I know nothing about parathyroid adenomas , or vit D/calcium.... but found this very useful site recently .. very good for explanations of all things parathyroid/calcium ...(and i do seem to vaguely remember something about a situation when you shouldn't give Vit D )parathyroid.com/

So you might find something helpful there.
