I can't remember if I've posted this article before, but Frontiers have just told me it has been published in full, so I suppose I haven't posted it.. The reference is:
Combination Thyroid Hormone Replacement; Knowns and Unknowns
THIS ARTICLE IS PART OF THE RESEARCH TOPIC
Combination Therapy for Hypothyroidism
Peter N. Taylor*, Vinay Eligar, Ilaria Muller, Anna Scholz, Colin Dayan
It's freely downloadable. It discusses what's known and what's not about combination therapy, but from the old standpoint often, though it does acknowledge our questioning of the use of TSH as a controller of therapy.

Very interesting article - thanks for posting it. It demonstrates just how much endocrinologists don't know about the thyroid system.
One thing I noticed was Reference 87, a study conducted by Graham Leese et al (Tayside) with the following conclusion:
For patients taking long‐term liothyronine we did not identify any additional risk of atrial fibrillation, cardiovascular disease or fractures. There was an increased incident use of antipsychotic medication during follow‐up.
In this study, 400 patients on T3 only were compared with 33955 patients on T4 only.
Thank you so much for posting. I'm changing GP's soon. Current GP is very sympathetic and prescribes T3 however future GP's stance on this currently unknown!
