Central/ secondary hypo?

Please could anyone give me Some advise. My bloods (in Italy) were :

FT3 2.29 pg/mL. (2.2 -4.2)

FT4 0.75 ng/dL (0.8. -1.7)

TSH 2.49 mUI (0.3-3.6)

What is the significance of this? Does normal TSH but low FT4 and borderline line low FT3 indicate pituitary problems and secondary hypothyroidism? What treatment is needed?

Yes, it very much does suggest Central hypo - could be a pituitary problem (secondary hypo) or a hypothalamus problem (tertiary hypo).

Treatment would very much depend on the results of further testing. It depends on whether it's just the TSH affected or all the other pituitary hormones. So, they would have to be tested, as well. But, as far as the thyroid is concerned, it would just be thyroid hormone replacement - levo and such - like primary hypo. There's not much else can be done.

Thank you, that’s what I suspected. About fifteen years ago I had an MRI brain scan following a mild stroke and at the time it also picked up a micro adenenoma on the pituitary gland so perhaps I need to get that checked out again.

This is from TUK which might be helpful for you. You will notice it is different from the ones doctors/endos use.

Quite similar to my results on diagnosis 10 years ago. Levothyroxine did not resolve for me and I am now being overseen by an NHS Endo on combination therapy now and feel well. I also found that I have a faulty DIO2 gene. I have to source the T3 myself sadly.
