NHS blood request form

Does anyone know if there is a time limit to using the blood request forms from GPs? I've one from around March 17 and wonder if the hospital walk in blood clinic could have an issue if I went for a test with it?

Many thanks!

With my hospital diabetes endocrinologist who I see yearly I get a blood test at the end of one appointment to do just before the next one so affectively a year old form.

I would be surprised if it was an issue......but it is the NHS so who knows.

If it's not too much of a pain to attend, personally I would give it a whirl. I suspect it will depend on who deals with it on the day and I doubt they even look at the date!

I’m a phlebotomist and we will take any form irrelevant of the date. However is the test still relevant and is the clinical information accurate? We have had forms that are are a few years old and have stated abdo pain or similar as the reason! Not entirely relevant anymore
