Is it possible that you have a cyst/nodule on the thyroid with no clear pathology? The consultant is looking to rule out "sinister pathology" (could be several things) so seems to be doing the right thing by requesting a timely FNA and ultrasound examination with any necessary staging according to BTA guidelines and the imaging of the thyroid nodules/cysts.

Under Relevant History:
"Thyroid nodule / cyst ^^ USS"
which means your previous history from a thyroid scan showed nodule or cyst, and the drawing notes indicate no clear pathology.
Under Clinical Question:
"to r/o sinister pathology"
which means to rule out sinister pathology for the nodule/cyst.
Under examination requested:
"USS Thyroid / BTA Guide / FNAC +/-"
which means a Thyroid Ultrasound using BTA guidelines with or without fine needle aspiration cytology.
